Nothing in this subpart shall be construed as limiting the inspector from making such tests or inspections as he or she deems necessary to be assured of the safety and seaworthiness of the vessel. Vessel did not complete a MOB and Fire Drill during their COI.

Multiple lighting fixtures on the freight deck were inoperative, damaged, and missing hardware.

All automatic control, remote control, safety control, and alarm systems must be failsafe. Vessel throttle head is displaying a code that indicates that that the Woods Hole side of the ferry main propulsion is having an issue with the reduction gear. The cause of the fault alarm is undetermined. Make permanent repairs to the reduction gear/alarm system.46CFR62.31(b)

Vessel had the #1 generator go off line due to a failure of the crank case sensor. #2 generator displayed a low hertz reading.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules (incorporated by reference, see 46 CFR 58.03-1), except as otherwise provided by this subchapter. Crank case sensor wire harness clip on the #1 Generator prime mover is worn and loose, resulting in an improper connection between the wire harness and engine. The improver connection has and will result in the shut down of the #1 generator. Replace the harness/securing clip to the crank case sensor wire harness.

If a main transverse watertight bulkhead is penetrated, the penetration must be made watertight. A thru bulkhead penetration in way of WTD #4 was improperly installed and not watertight. Provide watertight installation to the satisfaction of the attending Marine Inspector.

At least one-half the total number of lifebuoys on the vessel must each be fitted with a self-igniting light approved under approval series 161.010. The self-igniting light fitted on top side ring buoy was in-op. Corrected on the spot.

An MMC, license, MMD, COR, or STCW endorsement must be retained by the mariner to whom it was issued and, while valid, must be produced to verify qualifications when requested by an authorized official as identified in 33 CFR 101.515(d). The vessel Chief Engineer was unable to produce an MMC. Vessel was able to locate a properly credentialed Chief Engineer to take over operations over crew change-out.

Electric installations on vessels must ensure: Protection of passengers, crew, other persons, and the vessel from electrical hazards. Loose control junction box in way of WTD #1. Corrected on the spot.

Each vessel certificated under sub chapter H. must have an amplifier type announcing system that will supplement the general alarm system. Vessel master reported smoke from the public address system in the electrical control room. The PA system has been electrically isolated and tagged out. Affect repairs to the satisfaction of the Coast Guard Marine Inspector. Vessel authorized to continue operations with the following stipulations; No passengers are authorized on the freight deck or the 01 deck while transiting. All passengers shall be on the 02 deck with four crew members with radios, two of which must have bullhorns forward and aft on the 02 deck. Maximum number of passengers allowed is 350. A fire watch must be posted at the electrical control room.

While trouble shooting the inoperable public address system it was noted that the installed fire alarm (46CFR76.01-5) and required general alarm (46CFR77.05-1) are no longer operating. Vessel shall complete permanent repairs prior to carrying passengers. Vessel is authorized a one time transit from Martha's Vineyard, MA to Woods Hole, MA with no passengers or cargo.

Several sections of fire insulation on the car deck ceiling required to be re-taped and sealed. There are two sections of missing fire insulation around the newly installed AC piping in the air handler room. Make permanent repairs to structural fire protection insulation.

Several A-60 doors in the ladder well had holes from previously installed automatic closure devices that have been removed. The required A-60 doors are no longer considered A-60 with the holes in them. Make permanent repairs to the holes in all A-60 doors.

Periodic Safety tests must demonstrate the proper operation of the primary and alternate controls, alarms, power sources, transfer override arrangements, interlocks, and safety controls. Vessel's PSTP does not include written procedure for testing SSDG reverse power relays.

A survey, either general or partial, according to teh circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel. STBD Beam has damage from pier piling. Repair prior credit credit dry dock.

Vessel reported that the #3 SSDG tripped off line, and the #2 SSDG came on line as designed and took the load. Determine cause and repair the the satisfaction of the Coast Guard Marine Inspector.

The material, design, construction, workmanship, and arrangement of the main propulsion machinery must be at least equivilent to the stds. established by the ABS Steel Vessel Rules. Vessel's woods hold end shaft bearing coupling had 3/4 bolts looken and cause vibration. 46 CFR 58.05-1

Make permanent repairs to port side MES door. Vessel restricted to 50% capacity until repairs are made to the satisfaction of the attending USCG Marine Inspector.

STBD MES door was damaged during departure. Door must be replaced In-kind.

SSDG #2 VOLTAGE REGULATION AND PARALLEL OPS MUST MEET ABS STEEL VSL RULES 4-8-3/3.13.2, 3.13.3. SSDG 2 REACTIVE LOAD SHARE AND REVERSE POWER RELAY NOT OPERATING PROPERLY. LOCK OUT TAG OUT AT ALL TIMES UNDERWAY. SHORESIDE TESTING AUTH WHILE MOORED TO DOCK. SSDG 1 AND 3, SUITABLE FOR SVC.

Rescue boat davit does not meet the gravity lowering speed requirements as required by 46 CFR 199.153. Prove proper operation of rescue boat davits and provide measurement from davit to waterline.

Repair the following fire doors that do not close and latch properly. 02-35W-1, 2-18M-2, 2-25W-2, 2-35M-1

Provide signs for type and proper numbers of lifejackets for stowage locations as discussed.

Vessel reported inoperable #1 steering pump on bow steering gear (used on Woods Hole to Vineyard route). Vessel is required to effect reapirs & prove satisfactory operation to USCG prior to departure from Wood's Hole.

Prove proper operation of the #1 WTD to the satisfaction of the attending USCG Marine Inspector.

The vessel owner/operator must obtain an approved NTVRP Approval Letter by 14 Dec 2019.

Vessel's #2 rescue boat davit limit switch is not properly working. This is the switch located on the main crank. Repair or replace within 7 days.

Provide evidence on what employees are considered HazMat Employees, who has completed HazMat training IAW 49CFR176.13 and 172.704, and provide records of such training.

Prove proper operation of MV end bow thruster compartment bilge suction.

Repair lights that are out in MV end steering compartment.

Prove proper operation of rudder angle indicators in both steering compartments. Indicators are hanging up and require pounding on gages to get needles to move.

Label flashing red strobe lights to indicate purpose. ie, discharge of fixed fire extinguishing system for the bow thruster engine compartments.

Repair exhaust leak on #3 SSDG.

Prove proper operation of the ground detection system ammeter on the main switchboard.

Prove proper operation of the high lube oil temp alarm on the #1 SSDG.

Vessel's Woods Hole end exterior MES evacuation light on port side does not work. Vessel is lock out and tag out breaker for daylight operations until repaired.

Vessel is to inspect all exterior weather deck evacuation lighting plugs and J-boxes for functionality.

Provide plan approval for submitted A/C one line, load analysis, and coordination study.

Remove/disconnect not used R/G bow thruster sum alarm and bow thruster ctrl alarm from EMS Marcon panel.

Lockout one generator in accordance with Steamship Authority's proposal dtd 28Mar2017 until approval of short circuit calculations is obtained from the Marine Safety Center or obtain plan approval and implement permanent modifications so that only 2 generators can be paralleled at any given time. Permission must be obtained from the OCMI to remove lockout on generator. See uploaded 835.

Steering system alarms for the Woods Hole end steering system do not sound in the pilot houses. (power failure, phase failure, low oil level)

#3 SSDG low oil pressure shutdown/alarm not functioning. Repair and retest.

Calibrate rudder angle indicators in steering compartments and pilothouses. (indicators off by up to 7 degress)

Repair #2 sliding water tight door. Hydraulic leak from fill/vent cap when attempting to close using manual hand pump.

AVH01:Engineering department shall asses the bilge high level alarm in the Vineyard Haven bow thruster space. Due to the water reaching a height over the affected area it is recommended that the bilge high level alarm be lowered. Engineering shall evaluate the area and make effective repairs.

Engineering departments policy on conducting rounds of unmanned spaces should be reviewed ensuring these spaces are being checked often within SSA written policy. In addition, it is recommended upper management look into these rounds as well to make certain each department is doing its due diligence of checking spaces. After review has been made contact CG Inspector with corrective actions.

AVH03:Remove all excess water from Vineyard Haven bow thruster room.

AVH04: Provide a repair proposal that will include a plan to eliminate dissimilar metals for the shaft tunnel and salt water cooling line. In addition, due to this being a re-occurring issue the engineering department shall provide this plan in a timely manner.

Oil was found in the vessel's bilge areas in way of the main engines in the main engine room. Vessel is to remove oily water within the bilge.

Vessel's lifebuoys need to be restenciled with vessel's name and hailing port. Check all lifesaving for proper labeling.

Vessel's fire station was not properly labeled with station number. Ensure all stations are properly labeled.

Vessel's forward starboard side anchor cannot be recovered if anchor is released. Recovery motor does not operate as designed. Repair or replace to CG satisfaction.

Revise PSTPs DTD Feb 2016 to reflect actual test procedures utilized by the vessel and re-submit to MSC for review.

Lockout/Tagout on SSDG from being tied into the switchboard until MSC approves short circuit calculations.

Dry chem extinguisher was used for the emergency generator fire on this date. Replace in kind or recharge prior to operations. Location adjacent to e-gen space hatch.

Batteries, (x2 6D), wire, starter failed during manual mode starting causeing fire to the associated system. No other damage sighted in the vicinity or to the space. Prior to operations replace inkind.

M/V pilothouse has no control of steering system on the M/V end.

Mark with "E" on emergency light fixture fwd center in the E/R.

Replace bulbs on emergency lights in the following locations. 1) Fwd Center E/R 2)Over W/H bow thruster controller 3) Top of ladder in crew quarters 4)ECR

Prove proper operation of overspeed device on #3 SSDG.

Prove proper operation of time delay for the emergency generator space. Time delay failed.

Prove proper operation of the fire pump.

Adjust the following doors FSD 01-30W-01 and 2-25W-2 and doors 02 level port forward and 03 levelstbd aft.

Properly replace 5 saftey glass windows (03 level port fwd (All 3 windows), 03 level stbd aft fwd window and the 02 level stbd aft side, forward window.

Repair the Chart plotter M/V end

Renew reflective tape on forward rescue boat

Perform maintenance on anchor/windlass and retest
