Machinery alarms for the main engines must be tested twice in 5 years with no more than 3 years between tests. Prove proper operation of the low lube oil pressure and high jacket water temperature alarms to the attending marine inspector

Exposed hazards such as rotating machinery must be protected by guards. The alternator and bilge pump pulleys on the front of the main engines are exposed and not protected, provide guards.

Where crew may be employed there must be two means of escape. The secondary means of escape from the engine room is blocked by a seat cushion on the bridge. Provide a means to allow free exit and mark to ensure it is not blocked.

Replace dented HAD and install engine/power ventilation shutdowns on installed fire-extinguishing system in the engine room. Current setup on-board is not set up in accordance with the CG approved Manufactures Design, Installation, Operation and Maintenance Manual. Final testing need to be witness By USCG Marine Inspector prior to next operating season.

Replace batteries in both Life ring buoy lights with approved ones.

Replace Expired hydrostatic release on EPRIB,

Each watertight hatch must be marked on both sides in clearly legible letters at least 25 millimeters (1 inch) high: “WATERTIGHT DOOR—KEEP CLOSED” or “WATERTIGHT HATCH—KEEP CLOSED”. The watertight hatches to the lazerette, machinery space, and forward storage space were not marked. Properly mark watertight hatches. 46 CFR 140.160(e)

Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked: Spaces protected by carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.” Spaces into which carbon dioxide might migrate — “CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. DISCHARGE INTO NEARBY SPACE CAN COLLECT HERE. WHEN ALARM OPERATES VACATE IMMEDIATELY.” The machinery space and forward storage space were Carbon Dioxide can collect were not marked, properly mark entrances to spaces. 46 CFR 136.110 46 CFR 78.47-11(b)-(c)

Each ship subject to the Bridge-to-Bridge Act must have on board a radio operator who holds a restricted radiotelephone operator permit or higher class license. Vsl doesn't have a desiganted installed fire pump.

There are exposed wires on he port side aft bulkhead near the overhead.
