Each emergency generating set arranged to be automatically started shall be equipped with starting devices approved by the Administration with a stored energy capability of at least three consecutive starts. Primary start for the emergency generator is a battery bank; the batteries failed and were removed from service 27 days ago. Secondary means to start the emergency generator is by manual hydraulic means and cannot operate automatically. The vessel has since called 5 ports but has not taken any mitigating actions or manned the emergency generator space while the primary means of starting is disabled.74 SOLAS (14) II-1 44.2, Action: 30ABC

The company is responsible for ensuring that adequate resources and shore-based support are provided to enable the designated person or persons to carry out their function. The following situations created a hazardous environment and the company did not allocate the DPA with sufficient resources to restore shipboard safety. 1. Primary start for the emergency generator is a battery bank; the batteries failed and were removed from service. Company has failed to provide new batteries after receiving the vessel's urgent request on August 11th, 2018. For the last 5 port calls, the vessel has been operating without the critical safety function of an automatically starting emergency generator. 2. Company SMS manual requires repairs to the lifeboat to be conducted IAW the Manufacturer's Manual which states 'damages to highly strained or stressed parts shall be repaired by the Original Equipment Manufacturer'. A repair was completed by the vessel's crew on the port stern of the free fall lifeboat adjacent to the transom, below the waterline. Therefore, repair was not in accordance with the Manufacturer's Manual. ISM A/3, Action: 17AC
