The Master could not produce the valid COD during the inspection. This document must be on board the vessel. The TPO shall verify its placement on the vessel.

The VHF–FM radio must be installed at each operating station and connected to a functioning battery backup. The Master was unable to conduct a radio test due to a lack of battery backup for the radios. The battery backup shall be renewed and tested to the satisfaction of the TPO.

All portable fire extinguishers, semi-portable fire-extinguishing systems. . .must be inspected or tested at least once every 12 months, as prescribed in paragraphs (a)(1) through (8) of this section, or more frequently if otherwise required by the TSMS applicable to the vessel. The portable fire extinguisher on the bridge had not been inspected since 2020. The extinguisher shall be inspected and certified to the satisfaction of the TPO.

Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. These devices must be suitable for the expected route. The gasket on the bow compartment main deck hatch is deteriorated. Vessel shall renew the gasket to the satisfaction of the TPO.

Piping and machinery components that exceed 220 °C (428 °F), including fittings, flanges, valves, exhaust manifolds, and turbochargers, must be insulated. . . The exhaust manifolds and turbochargers on both main engines and both generators are unlagged and easily accessible. The equipment shall be lagged and insulated to the satisfaction of the TPO.
