Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must: (1)be maintained to ensure proper operation; (2) Be suitable for route and service; and (3) have suitable propulsion controls to provide the operator full control at each operating station. Vessels center propeller failed and disconnected from the shaft. Vessel is authorized for a one-time transit from current location ICWE MM 7 to the repair facility Main Iron Works ICWW MM 50 in accordance with the submitted transit plan. No further vessel movement is authorized until TPO and the Coast Guard deems the inspection of the repairs satisfactory.

The safety of the towing vessel is the responsibility of the master and includes compliance with the TSMS, if one is applicable to the vessel. After the crew noticed one of their barges in tow began listing and taking on water, the vessel master did not report the marine casualty immediately to the local USCG Command Center, but rather reported it 7 hours later. This not only violates the Company's TSMS Incident Reporting Policies/Procedures, but marine casualty reporting - 46CFR4.05-1. 1) The master of this vessel that was on duty at the time, and the mate (pilot) if on board, both need refreshing training on the TSMS and USCG reporting procedures, to be verified by upper management. 2) The TSMS Incident Reporting Policies/Procedures alludes to contradiction and limitation of the masters ability to report potential or actual marine casualties. Third bullet of Section 6.1 states the DPA makes the decision whether the USCG is notified, yet the time between an accident/casualty and the DPA approval may have prolonged lapses and lead to late USCG reporting. From conversations after the casualty, the DPA was notified 5 hours later. Clarify these TSMS sections as soon as possible with review of the TPO as well.
