A survey shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made and that the material and the workmanship of such repairs or renewals are in all respects effectively made; vessel sustained a 8"x10" hole to the port side shell plating, causing a hole also in one of the ship's fuel oil service tanks. Vessel affected temporary repair by welding a 2'x20"x18" steel box, with 1/2" plate thickness; Vessel is to transit from Port Fourchon, LA to Mobile, AL, without passengers or cargo, to affect permanent repairs to the satisfaction of the attending Marine Inspector. Vessel is not to return commercial service until permanent repairs have been satisfactorily completed and inspected to the satisfaction of the attending Marine Inspector. Vessel is to conduct soundings of the tank and to trim the vessel to where the temporary repair remains above the waterline.

The company should establish procedures for the implementation of corrective action, including measures to prevent recurrence. Materiel deficiencies noted during COI inspection on 23MAR21 to 29MAR21 resulted in a flag state detention; including deficiencies for inoperative rescue boat motor, fire dampers, sliding watertight doors, fixed CO2 time delays, and vital system automation. As well as missing rescue boat inventory control and associated work order with in NS5. An additional internal audit is required to determine the continued lack of maintenance to the vents, the results of which to be submitted to ABS and USCG, ABS to verify the adequacy of the proposed corrective action.

Means shall be provided for closing all other openings to the space protected from outside such space. Fire damper on the inboard starboard pump room ventilation trunk is severely deteriorated. Fire damper on the inboard port pump room ventilation trunk is warped and unable to close correctly due to the missing flat bar. The fire dampers on starboard side engine room off of the mechanical ventilation system do not fully close. Demonstrate proper operation of all fire dampers.
