A survey shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made and that the material and the workmanship of such repairs or renewals are in all respects effectively made; vessel sustained a 8"x10" hole to the port side shell plating, causing a hole also in one of the ship's fuel oil service tanks. Vessel affected temporary repair by welding a 2'x20"x18" steel box, with 1/2" plate thickness; Vessel is to transit from Port Fourchon, LA to Mobile, AL, without passengers or cargo, to affect permanent repairs to the satisfaction of the attending Marine Inspector. Vessel is not to return commercial service until permanent repairs have been satisfactorily completed and inspected to the satisfaction of the attending Marine Inspector. Vessel is to conduct soundings of the tank and to trim the vessel to where the temporary repair remains above the waterline.

The safety management system should include procedures ensuring that non-conformities, accidents and hazardous situations are reported to the company, investigated and analyzed with the objective of improving safety and pollution prevention. The company should establish procedures for the implementation of corrective action, including measures to prevent recurrence. Materiel deficiencies noted during COI inspection on 23MAR21 to 29MAR21 resulted in a flag state detention; including deficiencies for inoperative rescue boat motor, fire dampers, sliding watertight doors, fixed CO2 time delays, and vital system automation. As well as missing rescue boat inventory control and associated work order with in NS5. An additional internal audit is required to determine the continued lack of maintenance to the vents, the results of which to be submitted to ABS and USCG, ABS to verify the adequacy of the proposed corrective action.

Each hopper dredge must have sliding watertight doors (Class 3) approved under §170.270. Sliding water tight doors do not meet manufacturers requirement for sealing. A risk analysis and mitigation plan has been submitted to Sector Charleston.

Minimum safety trip controls required for specific types of automated vital systems are listed in Table 62.35-50. Vital Automation alarmed and shut down, but not at the prescribed temperature, speed or time requirements for MDE and SSDG alarms.

While conducting automation checks both starboard low lube oil pressure shutdown failed and port high lube oil temprature alarm failed. vessel to conduct self-verifcation of and attest to proper operation of all automated vital systems to the satisfaction of attending marine inspector in accordance with 46 CFR 61.20-3 and 46 CFR 61.40-10

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Vessel granted an equivalency based on the installation of a USCG approved 160.156 series rescue boat. Vessel informed company of engine issues with rescue boat on 22Mar2021. Rescue boat engine will shut down after being run for 6 to 8 minutes. No notification was made to the USCG regarding issue. Prove proper operation of rescue boat.

During preparation, launching, and recovery, each survival craft and the area of the water into which it is to be launched or recovered must be adequately illuminated by lighting supplied from the emergency power source. Starboard side emergency light at the #1 life raft does not work. Prove proper operation of light.

Each hopper dredge must have sliding watertight doors (Class 3) approved under 46 CFR 170.270. 7 out of 10 sliding watertight doors would not seal as designed by manufacturer. Prove proper operation of all sliding watertight doors.

In regards to the fixed fire suppression system, systems of the type indicated in Table 95.15-5(d), other than systems for tanks, which are of more than 300 pounds of carbon dioxide, shall be fitted with an approved delayed discharge so arranged that the alarm will be sounded for at least 20 seconds before the carbon dioxide is released into the space. Both port and starboard machinery spaces main CO2 systems time delays failed. Systems serviced multiple spaces, not limited to engine rooms and steering gear rooms. A satisfactory function test of all fixed suppression systems must be conducted.

Where mechanical ventilation is provided for spaces protected by a carbon dioxide extinguishing system, provisions shall be made so that the ventilation system is automatically shut down with operation of the system to that space. And where natural ventilation is provided for spaces protected by a carbon dioxide extinguishing system, provisions shall be made for easily and effectively closing off the ventilation. Means shall be provided for closing all other openings to the space protected from outside such space. Fire damper on the inboard starboard pump room ventilation trunk is severely deteriorated. Fire damper on the inboard port pump room ventilation trunk is warped and unable to close correctly due to the missing flat bar. The fire dampers on starboard side engine room off of the mechanical ventilation system do not fully close. Demonstrate proper operation of all fire dampers.

Vertical ladders and deck scuttles shall not in general be considered satisfactory as one of the required means of escape. However, where it is demonstrated that the installation of a stairway would be impracticable, a vertical ladder may be used as the second means of escape. Assist spring is inoperable on the emergency escape hatch from the port engine room, prohibiting safe egress during an emergency.

Identified holes in deck/starboard engine room overhead forward of the storage room. Descale and UT identified areas and provide repair proposal.

Found fwd mast on bow damaged, crop renew and repair mast. Found aft mast foundation deteriorated, crop renew to good steel. 3rd frame fwd stbd side hopper coaming waisted, crop and renew. 46 CFR 90.35-1

Found piping to fire stations #24 and #25 wasted. Descale and provide UT report attesting to the satisfactory condition or crop and renew.

CO2 system in Bosun Locker is not outfitted with efficient time delay. Outfit bosun locker CO2 system with satisfactory time delay. CO2 control station must be manned while entering the space.

#2 fuel shut off light in starboard tunnel is extinguished. Replace as new.

Found general alarm bells in starboard tunnel inoperable.

Found wastage in the following areas: Pitting and doublers IWO port bow deck, port and starboard well deck lockers, mast foundation, 3rd frame forward starboard side hopper coaming. Submit repair proposal.

Forward bow ladder and walkway damaged. Repair or remove to the satisfaction for CG marine inspector.

Found bracket for semi-portable fire extinguisher outside starboard steering room broken. Repair to the satisfaction of CG marine inspector.

Vessels may retain the number, type, and arrangement of lifesaving appliances previously required and approved for the vessel as long as the arrangement or appliance is maintained in good condition to the satisfaction of the [name withheld] review of the ongoing repairs to the lifeboat davit, USCG Sector Mobile has determined that the davit is not in good condition and is no longer serviceable. As such, the davit must be replaced in accordance with regulations. Prior to any work, provide USCG Sector Mobile a replacement plan for approval.

Unacceptable repairs and maintenence on open lifeboats: Complete repairs & maintenance, including FRP, engine, & retroreflevtive tape, completed with materials & processes per lifeboat manufacturer's specifications. This requirement issued for 30 days while vessel operates with an alternative lifesaving arrangement approved per 46 CFR 199.20(d).

Vessel unable to launch port lifeboat due to mooring arrangement. Make available for launch.

Starboard Steering Gear loss of power alarm mis-labeled. Label correctly.

Starboard main diesel engine turbo intake non-metallic flex coupling has been replaced with a rubber gasket material. Replace with proper OEM part.

Starboard Lifeboat davit, aft post, fwd roller axle hub has wastage. Clean and gauge wasted area and provide report and repair proposal.

OWS does not have proper testing procedures from manufacturer. Provide procedures onboard.

Electrical extension cord is run from lighting junction box. Remove cord connection.

Vessel has identified crack in the forepeak and will conduct permanent repairs.

Both port/stbd lifeboat blocks show chaffing damage from falls on all four blocks in eight areas. The wire ropes for the falls were not greased as per manufacture's instructions.

Replace safety relief valve on port/aft ship service air receiver (MAWP 110 psi).

Port side, forward steering gear ram is leaking from two fittings that are actively leaking during operation.

Starboard side, forward steering gear ram is leaking from the oil seal while operating and also while secured.

Starboard fire pump is inoperable due to grounded pump motor.

Clean/pump out stbd dredge pump and pump engine room bilges (2 spaces).

Remount electrical box in port dredge pump engine room.

Repair general alarm outside paint locker.

Check all stern navigation lights for painted lens.

Vessel’s starboard main engine turbo failed and does not meet minimum standard of material, design and construction of main propulsion machinery as required by 46 CFR 58.05-1. Owner must provide plans to correct engine turbo to Class and Coast Guard Marine Inspector for approval. Vessel has 15 days to correct to the satisfaction of Class and the attending Coast Guard Marine Inspector.
