The machinery, boilers, and other pressure vessels associated piping systems and fittings shall be of design and construction adequate for the service for which they are intended.PSCO observed Jacket Water leaks at number one and five main engine cylinders. When jacket water valves were opened the leak was obeserved as severe. C/E stated the O-rings had failed and S/F could replace. 74SOLAS(04) II-1/26.1

Resolution: Surveyor went on board to verify Jacket water cylinders were no longer leaking. Verified new O-rings were installed and no leaks were observed at the time of attendance.
