The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules. Vessel reported that it lost RPMs on the Port Engine resulting in a reduction of propulsion. Vessel is authorized a one-time transit, with two assist tugs, from Anchorage 23 to Valero, BNC 4 Pier. Conduct repairs to the satisfaction of the Marine Inspector and ABS. Provide MI's with a satisfactory technician report. 46 CFR 96.03-1(a) 46 CFR 58.05-1(a)

The master, mates and engineers on any vessel to which this section applies must hold a license or MMC officer endorsement to serve in that capacity issued by the Coast Guard under parts 10 and 11 of this subchapter. Vessel had an QMED/RFPEW filling the role and watch of a licensed engineer.46 CFR 15.701(b)

Approved portable fire extinguishers and semi-portable fire extinguishing systems must be installed in accordance with Table 95.50-10(a) of this section. Vessel found missing fire extinguisher for the paint lockers. 46 CFR 95.50-10(a) U.S. Supplement 3.11.21/3.1.2.10

Escape routes shall be maintained in a safe condition, clear of obstacles. Emergency escape on the aft bulkhead of the engine room found obscured by storage of ladders and other items. 74SOLAS (04)II-2/13.1.2

Vessel is required Periodic Safety Test Procedures due to automation and reduction of vessel's required manning. Vessel's PSTPs need to be updated to remove thermal fluid heater and modify the set points of several test, actual procedures remain valid. Due by next dry dock.

Vessel has bridge installed quick release lifebuoys with self activating smoke signals to comply with SOLAS. Lifebuoys were not of approved type for quick release. Vessel replaced with heavier approved lifebuoys.

Interviews of the tug crew regarding vessel operations were conducted on July 18, 2014 and a limited examination of the Tug VISION Safety Management System (SMS) was conducted. Non-conformities and Major non-conformities with ISM Code were discovered. Complete an external audit of the Tug VISION's SMS and provide copies to the OCMI Portland, Oregon. SOLAS IX Regulation 3.1 and 33 CFR 96.340

The forward engine room escape door does not meet the requirements of a fire door. The WTD is not quick-acting (6 dogs), nor is it a standard self closing fire door. Change the door so it opens quickly to provide a fast escape in an emergency, and equip the door with a closing device. SOLAS II-2 Reg 13.3.1.5, II-2 Reg 13.4.2.1, NFPA 12 and NVIC 9-97 Ch 1

Provide proof that the two watertight doors in the Intercon space meet A-0 standards, FTP Code and are approved by ABS and USCG. The "as built" plans show the doors to be from CEN-TEX Marine. However, the current doors are not CEN-TEX doors and per the manufactures review. All watertight doors on the vessel are now in question, as the doors do not meet as built plans. SOLAS II-2 Reg 9 4.2.1

The main deck quick acting watertight doors leading to the Intercon space (forward and aft) have fixed (bolted) bronze portlights installed. These doors are required to meet A-0 fire door standards. Provide proof that the bronze portlights and covers meet A-0 fire standards. SOLAS II-2 Reg 9 4.2.1 and FTP Code.

Excessive fuel found in clean leak-off pipe on port MDE. Locate/repair fuel system malfunction tp the satisfaction of the attending class surveyor [name withheld] surveyor's report to OCMI.

Electronic engine control for the starboard engine reported inoperative.

Due to failure of port shaft CPP automation, the engine room is required to be manned with a licensed engineer at all times during operation of the port main engine.

Automation from wheelhouse for the port shaft CPP system is not functional, but the CPP can be operated in "safe mode". Prove proper operation of the port shaft CPP.

Prove proper operation of port main diesel engine.
