Piping systems used where the fluid temperature exceeds 150 °F must be suitably insulated as necessary to preclude injury to personnel. MI observed sections of the Thermal Fluid Heater piping not covered in insulation. Install insulation for crew protection to the satisfaction of the MI. 46 CFR 56.50 (k) 16c Vessel given 14 days to complete insulation on thermal piping. Missing small section in aft of the vessel piping.

The master shall ensure that the following chemical protective clothing constructed of materials resistant to permeation by the cargo being handled is worn by all personnel engaged in an operation. MI Observed no Chemical Resistant protection for handling of cargoes regulated in CFR 153. Provide Chemical protective clothing to the satisfaction of the MI. 46 CFR 153.933 (a) 701c

No cargo tank hatches, ullage holes, or Butterworth plates shall be opened or shall remain open without flame screens, except under the supervision of the senior members of the crew on duty, unless the tank opened is gas free. Marine inspector observed Butterworth plate unbolted and opened. 46 CFR 35.30-10 701c

Vessels in ocean, coastwise, or Great Lakes service, except unmanned barges, must be fitted with anchors. chains, and hawsers in general agreement with the Standards established by the American Bureau of Shipping, see 46 CFR part 90, subpart 90.35. Marine inspector observed wasted securing points on the S forward anchor windlass control. Repair to the satisfaction of the Marine Inspector. 46 CFR 96.07-5(a) 16c

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. When transferring cargo while fast to a dock or at anchor in port, the master shall ensure that the tank ship displays a warning sign at the gangway facing the shore so that it may be seen from the shore and another warning sign facing outboard toward the water so that it may be seen from the water. Each warning sign must have the following legends: Warning. Dangerous Cargo. No Visitors. No Smoking. No Open Lights. Marine Inspector observed no warning signs posted on the Port Side Manifold's. Each life jacket must have a whistle firmly secured by a cord to the life jacket. Marine Inspector observed whistle line attached break due to deteriorating line. Repair to the satisfaction of the Marine Inspector. 46 CFR 199.70 (b)(4)(ii) 16c

Each lifebuoy must be marked in block capital letters with the name of the vessel and the name of the port required to be marked on the stern of the vessel. Marine Inspector observed the Port Aft and Port amidships lifebuoy's without proper name of vessel. Repair to the satisfaction of the Marine Inspector. 46 CFR 199.70 (a)(2) 16c

When inspecting oil-burning vessels, either internal-combustion type or steam-driven type, the marine inspector shall examine the tank tops and bilges in the fireroom and engineroom to see that there is no accumulation of oil which might create a fire hazard. After examining the #2 (stbd) service generator, MI noticed bucket catching oil and a significant amount of oil accumulation surrounding the generator. Secure the source of oil leak and clean-up oil accumulation prior to cargo operations. 46 CFR 31.10-50(a)

Inerted means the oxygen content of the vapor space in a cargo tank is reduced in accordance with the inert gas requirements of 46 CFR 32.53. The master shall ensure that the inert gas system is operated as necessary to maintain an inert atmosphere in the cargo tanks. One IG combustion chamber reported inoperable; prior to discharge of cargo, IG combustion chamber to be repaired to satisfaction of Coast Guard Marine Inspector.

The installation of all electrical engineering systems must meet the requirements of Subchapter J. Class I, Division 1 locations on the weather deck may have only approved intrinsically safe or explosion proof equipment. Numerous electrical cables within the hazardous zone were found with the outer sheath deteriorated. Chief Engineer attested that these cables still have a satisfactory inner sheathing. Since the electrical cables do not meet the manufacturers specifications, the vessel shall complete temporary repairs of all deteriorated electrical cables to the satisfaction of the attending Coast Guard Marine Inspector. Permanent repairs will be completed at next shipyard but no later than 31MAY2025.

Samples which have to be kept on board shall be stowed in a designated space situated in the cargo area. The stowage space shall be cell-divided in order to avoid shifting at sea and equipped with adequate ventilation. The cargo samples were not cell-divided. Provide adequate division of cargo samples to the satisfaction of the attending USCG Marine Inspector.

All lifesaving appliances and arrangements on tank vessels must be in accordance with Subchapter W. Lifebuoys, including those with attached lights, must be so distributed as to be readily available on each side of the vessel. Each cargo vessel must carry the number of lifebuoys prescribed in Table 199.271. For vessels 492 feet in length but less than 656 feet in length, 12 ring lie buoys are required, however per Enclosure 2 of NVIC 02-81 CH 2, the vessel may be outfitted with half this number for a total of 6. Vessel currently has 3 ring lifebuoys on board with only one having an attached light. Install three more lifebuoys, to include another with an attached light, for a grand total of 6 lifebuoys with 2 attached lights.

Each alternating-current ship's service switchboard must have a voltage regulator and voltage regulator functional cut-out switch. Switchboard on starboard side of cargo control space on barge is missing manual voltage regulator. Prove proper arrangement of voltage regulator on switchboard.

The following location in the weather is a Class I, Division I (Zone I) location and may have only approved intrinsically safe, explosionproof, or purged and pressurized equipment: within 10 feet of a cargo tank ullage opening. Nav light batteries on forward part of the deck are approximately 06 feet from ullage openings. Demonstrate proper arrangement of batteries 10 feet from ullage openings and not in a hazardous location.

Vessel response plan required for unmanned tank barges carrying oil as primary cargo. Procedures in the order of priority for the crew to mitigate or prevent any discharge or a substantial threat of such a discharge in the event of the following casualties or emergencies: grounding and stranding, explosion or fire, hull failure, excessive list, equipment failure. Master and crew did not follow vessel's VRP or activate it upon notice of potential grounding. Master failed to do the following IAW the VRP and ISM: sound alarms, muster crew, take soundings, or call securite on the radio. Additionally, Master did not implement plans IAW company procedures as per document no. ATB-VOM-11. Properly install the VCS flange stud per 46 CFR 39.20-1. Studs must be permenently installed.

Vessel is prohibited from cargo operation (Loading or Unloading) until the port generator is re-installed and tested to the satisfaction of attending Marine Inspector and class (ABS).

Port service diesel generator was damaged due a fire. Vessel O/O was granted permission to make repairs @ this port, however generator could not repair by schedule departure. Vessel is permitted to depart port , proceed to lay berth (Texas City, TX) to re-install generator and test to the sat. of USCG and Class prior to cargo operations.

During the transit to the lay berth, Texas City, Tx, the vessel must remain in an gas free/inerted stated.
