Main switchboard indicated a ground fault. O/O shall investigate what equipment is causing ground and supply corrective action to the satisfaction of MI.

The fire pump breaker label on the switchboard is marked 35 Amp, the installed breaker is 100 Amp. O/O provide evidence that the breaker and conductors are appropriate for the 100 Amp or replace the breaker with a the correct rated breaker.

Pressure vessels over 5 cubic feet in volume and over 15 pounds per square inch maximum allowable working pressure (MAWP) must be equipped with an indicating pressure gauge and with one or more spring-loaded relief valves, to prevent pressure from exceeding MAWP. The relief valves on port and stbd air receivers did not lift as required during testing. O/O shall repair or replace relief valves and verify proper operation to the satisfaction of MI.

Survival craft must be stowed in accordance with the requirements of 199.130 of this chapter, as far as practicable on existing towing vessels. A survival craft that weighs 407.8 lbs or less may be lifted not more than 1 foot in order to launch. Each liferaft must be arranged to permit it to drop into the water from the deck on which it is stowed.
Current liferaft stowage/launching arrangements does not meet these requirements. Owner/operator to provide a proposal to the OCMI for modification and timeframe.
