On all tank vessels, where cargo pump shafts pierce gas tight bulkheads, stuffing boxes with readily accessible gas tight glands shall be provided. The starboard bellows that pierces the bulkhead between the starboard cargo pump prime mover and the starboard cargo pump is not gas tight. Rectify deficiency prior to operation of the starboard cargo pump.

Liferafts must be readily transferable for launching on either side of the vessel or supplemented with additional liferafts to bring the total capacity of the liferafts available on each side of the vessel to at least 100% of the total persons on board. Only one liferaft was installed on top of the accommodation space preventing side to side transfer.
