Operational tests and checks of all safety and limit controls, combustion controls, programming controls, and safety relief valves must be conducted by the owner, chief engineer, or person in charge at the inspection for certification, periodic inspection, and when directed by the Officer in Charge, Marine Inspection, to determine that the control components and safety devices are functioning properly and are in satisfactory operating condition. These tests and checks must be conducted in the presence of a marine inspector and must include the following: proper prepurge, burner ignition sequence checks, operation of the combustion controls, limit controls, fluid flow controls, fluid level controls, high temperature control, proper postpurge control, and verification of the flame safeguard. Vessel was unable to demonstrate the required thermal fluid heater tests and checks. Demonstrate the above tests/checks in the presence of a Marine Inspector prior to 21June2026. 46 CFR 61.30-20 705c

Grind and re-weld the following areas in the bow rake: 1. fracture on third longitudinal from bottom, stbd side 2. all welds on 3rd vertical beam from aft bulkhead on port side shell

Address the following areas prior to credit drydock: 1. Void 8S: cracked weld on the uppermost longitudinal where it meets the third vertical from aft. 2. Void 2S: cracked weld on the third longitudinal from the bottom where it meets the forward-most vertical. 3. Void 3S: cracked weld on the third from the bottom longitudinal where it meets the second vertical from the forward end of that space. 4. Void 8P: cracked weld on top longitudinal where it meets the number 1 forward-most vertical.

If, during an internal structural examination, damage or deterioration to structural members is discovered, the OCMI may require the vessel to be drydocked or otherwise taken out of service to further assess the extent of the damage and to effect permanent repairs. Six vertical sideshell frames in four voids had associated weld fractures. Repairs are required prior to executing voyage associated with SVLLE outside of LBS route, or within 30 days.

The inspection for certification shall include an inspection of the structure, boilers, and other pressure vessels, machinery and equipment. During TFH operation, found broken relay preventing fuel pump operation.

The bulkhead between the #1 and #2 void stbd, approximately between #1 and #2 side longs needs to cropped and renewed at the next dry dock. 46CFR32.60-1

Provide proof of testing of the cargo piping relief valves required by 33 CFR 156.170 prior to off loading cargo.

Obtain an approved Vessel Response Plan (VRP) for barge to conduct operations in Sector Lake Michigan's Captain [name withheld] (COTP) area required by 33 CFR 155 Subpart D.
