Following completion of Shaft replacement, the vessel back away from the dock when the shaft disconnected from the coupling, sliding back and lodging on the steering rudder. Propulsion machinery, including main engines, reductions gears, shafting, bearings, and electrical equipment must be maintained to ensure proper operation.
Prior to movement, assess extent of damages caused by this incident and affect permanent repairs to the satisfaction of the Coast Guard.

Decks and bulkheads designed to be watertight or weathertight must be maintained in that condition. There was an 2" open penetration found on the deck of the aft void and 12" of water found in the void. Install threaded plug in the open collar. If threads are too damaged to make a watertight seal, the use of a marine grade epoxy is authorized. If epoxy is used, permanent repairs will be required at next scheduled drydock. Remove water from void space and make space available for inspection to ensure there is no water intrusion from a breach in the hull or rudder seals.

Pressure vessels over 5 cubic feet in volume and over 15 pounds per square inch maximum allowable working pressure (MAWP) must be equipped with an indicating pressure gauge (in a readily visible location) and with one or more spring-loaded relief valves. The total relieving capacity of such relief valves must prevent pressure from exceeding the MAWP, as established by the manufacturer, by more than 10 percent.
Pressure vessel in engine room has a MAWP of 200 psi; the relief valve on the pressure vessel is set to relieve at 250 psi.
