WEST VIRGINIA

On 12 December 2020 at approx. 17:55 eastern time, IVO mile 800 Ohio River the ITV WEST VIRGINIA was underway downbound with 15 loaded hopper barges when it experienced multiple shutdowns of generators and MDEs. The incident bagan when the aft generator which was being used as primary shut down suddenly. The engineer on board the vessel tried to start the forward generator but it shut down after running for approx. 2 minutes. Soon afterward, the port and starboard MDEs shut down. Vessel was required to troubleshoot to determine the cause of the shutdowns and repair to the satisfaction of Coast Guard and TPO. Once repairs were completed vessel was required to contact Sector Ohio Valley marine inspector at [phone withheld] before placing vessel back into service.

Machinery systems must be maintained to provide for safe operation of the towing vessel and safety of person onboard. The vessel experienced a failure associated with the port engine's clutch. Repair to bring equipment back into good working order.

Port gear box air shaft misaligned causing gear box to operate outside of parameters. Machinery systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. Prove proper operation of Port Gear Box to the satisfaction of the attending USCG Marine Inspector. Vessel authorized to transit light boat to Point Pleasant, WV for repairs.
