No later than July 22, 2019, the owner or managing operator must implement a health and safety plan. The health and safety plan must document compliance with this part and include recordkeeping procedures. No health and safety plan meeting the requirements of 46 CFR 140 Part E onboard vessel. Vessel operator shall provide a copy of the vessel's health and safety plan meeting requirements listed in 46 CFR 140 to the OCMI. All deficiencies identified from this inspection shall be recorded and corrected IAW the health and safety plan to demonstrate compliance.

Carriage requirement. A towing vessel must carry a combination of day and night visual distress signals indicated in Table 141.370 of § 141.370 for specified areas where the vessel operates. Vessel had no visual distress signals onboard. Vessel limited to daylight operations until corrected.

Each watertight door and watertight hatch must be marked on both sides in clearly legible letters at least 25 millimeters (1 inch) high: “WATERTIGHT DOOR - KEEP CLOSED” or “WATERTIGHT HATCH - KEEP CLOSED”. Watertight hatches and doors are not labelled.

Any towing vessel with alternating watches (shift work) or overnight accommodations must identify the duties and duty stations of each person onboard during an emergency, including: (1) Responding to fires and flooding; (2) Responding to emergencies that necessitate abandoning the vessel; (3) Launching survival craft; (4) Taking action during heavy weather; (5) Taking action in the event of a person overboard; (6) Taking action relative to the tow; (7) Taking action in the event of failure of propulsion, steering, or control system; (8) Managing individuals onboard who are not crewmembers; (9) Managing any other event or condition which poses a threat to life, property, or the environment; and (10) Responding to other special duties essential to addressing emergencies as determined by the TSMS applicable to the vessel, if a TSMS is used.

Drug and alchohol testing program documentation and testing equipment not made available to attending inspectors. Provide proof of compliance with 46 CFR 4.06 to OCMI.

Charts, maps, and nautical publications for the intended voyage. Army Corps charts and light list out of date.

Each lifebuoy on a towing vessel must be marked in block capital letters with the name of the vessel. Lifebouy missing vessel name.

Each lifejacket must be marked: (1) In block capital letters with the name of the vessel; and (2) With Type I retro-reflective material approved under approval series 164.018. The arrangement of the retro-reflective material must meet IMO Resolution A.658(16) (incorporated by reference, see § 136.112 of this subchapter). (g) Lifejackets must have the following attachments and fittings: (1) Each lifejacket must have a lifejacket light approved under approval series 161.012 or 161.112 securely attached to the front shoulder area of the lifejacket. (2) Each lifejacket must have a whistle firmly secured by a cord to the lifejacket. (01) lifejacket missing name, (08) life jackets missing light and whistle. Vessel limited to daylight operations until corrected.

Each exhaust pipe from an internal combustion engine which is within reach of personnel must be insulated or otherwise guarded to prevent burns. Both generators missing insulation on exhaust manifolds and turbo chargers. Install insulation as required.

The lights prescribed in these Rules (Subpart C) shall have an intensity as specified in Annex I to these Rules (33 CFR part 84). All navigation lights were found to have household bulbs installed. Remove household bulbs and replace with bulbs made for fixture.

A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. Evidence was found that fuel oil is leaking from port fuel tank into port aft void space. Identify source of leak and report to USCG Marine Inspector before attempting repairs.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The following alarms must be provided - High bilge levels. Crew was unable to find bilge alarm sensor for testing. Test bilge system in the presence of a USCG Marine Inspector.

he master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. The following deficiencies were identified: • Port aft water tight hatch wasted/inoperable • Engine Room water tight door gasket damaged. Repair as required.

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. The medical certificate will be issued for various periods of time based upon the endorsements the mariner holds. Except as otherwise noted, medical certificates will be issued for the following periods of time: Medical certificates issued to all other mariners will be issued for a maximum period of 5 years. Vessel's Captain has an expired medical certificate (Expired in 2019). Present Marine Inspector with valid copy of medical certificate.

Vessels with square bow. For vessels having a square bow, the name of the vessel must be marked on some clearly visible exterior part of the bow in a manner to avoid obliteration. The name and hailing port must be marked on some clearly visible exterior part of the stern. Vessel name missing from bow. Install as required.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. The following deficiencies were identified: 1. Electrical cable 110v for port running light freighted. 2. Light switch enclosure on stack deck has exposed wires 3. Exposed electrical connections found on boiler in engine room 4. Exposed electrical connection found on heater in compressor room 5. Loose battery connections for main diesel engines 6. Breaker panel #1 in engine room missing cover closing device.

Extension cords must not be used as a permanent connection to a source of electrical power. Extension cords being utilized to power navigation lights and other permanent light fixtures on vessel. Remove extension cords and install permanent circuits with marine grade cable.

If twist-on types of connectors are used, the connections must be made within an enclosure and the insulated cap of the connector must be secured to prevent loosening due to vibration. Twist-on connector found unsecured and outside of an enclosure for light fixture in old rudder room.

No person may operate any vessel equipped with installed toilet facilities unless it is equipped with: (1) An operable Type II or III device that has a label on it under § 159.16 or that is certified under § 159.12 or § 159.12a. Vessel MSD found to be wasted and Bondo applied to tank walls. Repair or replace MSD with new.

All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. The following deficiencies were found: 1. Oil soaked rags found in engine room bilge 2. Fuel soaked sorbent pad found in port aft void. Remove fire hazards as required.

The master or officer in charge of a navigational watch must ensure all hatches and openings of the hull and deck are kept tightly closed. Port and Starboard aft hatches were found to be in the open position. Keep closed as required.

Flammable and combustible products must not be stored in machinery spaces, unless they are stored in a suitable container that meets the requirements of § 142.225 of this subchapter. Engine oil was found being stored in engine room. Remove flammable products from engine room.

All vessel equipment must be used in accordance with the manufacturer's recommended practice and in a manner that minimizes risk of injury or death. This includes machinery, deck machinery, towing gear, ladders, embarkation devices, cranes, portable tools, and safety equipment. Forward ladder wasted/holed. Crop out wasted steps and replace with new.

Vessel departed Guttman Oil Company without COTP authorization as per the COTP Order 14-05 after having the waste oil removed from the bilge.
