The following minimum information is required in the record: The dates when examinations were performed, the identification of each item of towing gear examined, and the name(s) of the person(s) conducting the examinations. Towing gear log did not have standard entries for condition of the gear. 46 CFR 140.820(b)

The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. Navigation assessments containing information in 140.635 were not available for inspection. 46 CFR 140.635

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space. Emergency light located on the stairwell leading to the pilot house was inoperable. 46 CFR 143.410(b)

The master must ensure that all hatches, doors, and other openings designed to be watertight or weather- tight function properly. Main deck port weather-tight door had wasted gasket and hardware missing from the top hinge. 46 CFR 140.610(b)

Electrical systems and equipment must function properly and minimize system failures. Front door for the breaker box located in the pilot house was found missing. 46 CFR 143.400(a)

For towing vessels with berthing aboard, a sufficient number of additional lifejackets must be carried so that a lifejacket is immediately available for persons at each normally manned watch station. Life jacket in the pilot house was missing. 46 CFR 141.340(b)

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford protection against hazards to life, property, and the environment. Deck area located outside of the pilot house had multiple areas of wastage. Close off access to that deck area until repairs are made during next haul out. 46 CFR 140.205(a)

Piping and tanks exposed to the outside of the hull must be made of metal and maintained in a leak free condition. Leaking supply piping noted on port steering hydraulic tank. Leak is located near the tank connection point beneath the valve. 46 CFR 143.270

Electrical system and equipment must function properly and minimize system failures and fire and shock hazards. Port engine room overhead light was no longer secured in protective covering and was hanging down from its wires. 46 CFR 143.400

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Identified signs of exhaust leak on port engine; provide root cause and corrective actions. 46 CFR 143.205(a)

The size, material, and condition of towlines, lines, wires, push gear, cables, and other rigging used for making up a tow or securing the towing vessel to a tow must be appropriate for: The static loads and dynamic loads expected during the intended service. Vessel’s towing wire on port side was frayed. Replace to the satisfaction of the MI. 46 CFR 140.801 (b)(2)

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. This may be accomplished by an installed detection system, or by using individual battery-operated detectors meeting UL 217. Detection systems or individual detectors must be kept operational at all times when the crew is onboard. Identified inoperable smoke detector in galley. 46 CFR 142.330(b)

Any towing vessel with alternating watches (shift work) or overnight accommodations must identify the duties and duty stations of each person onboard during an emergency, including: (1) Responding to fires and flooding; (2) Responding to emergencies that necessitate abandoning the vessel; (3) Launching survival craft; (4) Taking action during heavy weather; (5) Taking action in the event of a person overboard; (6) Taking action relative to the tow; (7) Taking action in the event of failure of propulsion, steering, or control system; (8) Managing individuals onboard who are not crewmembers; (9) Managing any other event or condition which poses a threat to life, property, or the environment. Vessel’s emergency duties list did not address crewmembers’ response to all required emergency scenarios. 46 CFR 140.405(b)

A towing vessel must carry a combination of day and night visual distress signals indicated in Table 141.370 of § 141.370 for specified areas where the vessel operates. Vessel had 4 distress signals onboard, which is less than amount required. 46 CFR 141.375(a)

Each lifebuoy on a towing vessel must be marked in block capital letters with the name of the vessel. Markings on main deck, port side lifebuoy were faded and require new stenciling. 46 CFR 141.360(b)(4)

Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure the following: All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Identified accumulation of oily water in bilge outboard of starboard main engine. Investigate source and clean affected areas to the satisfaction of the MI. 46 CFR 142.220 (a)

Windows and other openings at the operating station must be of sufficient size and properly located to provide a clear field of vision for safe operation in any condition. Identified crack in pilothouse window. Replace prior to next credit drydock. 46 CFR 144.905(a)

Existing equipment and installations.. may be continued in service so long as they are in good condition and accepted by the local [name withheld] is a pinhole in the discharge line for the fire main which must be repaired for the system to remain satisfactory to the OCMI. 46 CFR 142.215(d)

There must be an installed or portable bilge pump for emergency dewatering. All installed bilge piping must have a check/foot valve in each bilge suction that prevents unintended backflooding through bilge piping. The port engine room bilge suction valve is missing a handle. 46 CFR 143.275

A towing vessel must carry adequate and up-to-date charts, maps, and nautical publications for the intended voyage. The charts have not been updated since July 20, 2023. 46 CFR 140.705(b)

Closure devices must be provided for deckhouse or hull penetrations which open to the exterior of the vessel and which may allow water to enter the vessel. These devices must be suitable for the expected route. The engine room port hole on the port side cannot be secured due to a missing a wing nut. 46 CFR 144.320(b)

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space. The emergency light at the top of the ladderwell is not operational. 46 CFR 143.410(b)

Prior to getting underway for the first time on a particular towing vessel, each crew member must receive a safety orientation. 02 new crew members have not had their required safety orientation. 46 CFR 140.410(b)

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crew members on inspected vessels. The list for crew members enrolled in a random drug testing program is not up to date. 46 CFR 16.230(a)

All training required in this section must be documented in owner or managing operator's records. The health and safety training records for the crew are missing. 46 CFR 140.515(f)

Lifejackets must have the following attachments and fittings: each lifejacket must have a lifejacket light. There was 01 lifejacket missing a light. 46 CFR 141.340(g)(1)

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who occupy a position, or perform the duties and functions of a position, required by the vessel's Cerificate of Inspection. Although present in other vessels managed by the company, the vessel was missing proof onboard the vessel showing that the vessel's crewmembers were enrolled in a drug testing program. Vessel operator to provide proof of all necessary members being enrolled in the drug testing program. 46 CFR 16.230(a)(1)

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. The vessel's smoke detector was inoperable in the berthing area. 46 CFR 142.330(b)

The inspection will determine that the vessel is in possession of any valid certificates or licenses issued by the Federal Communications Commission, if required. Vessel is missing the FCC bridge to bridge certificate. 46 CFR 136.212(b)

All owners and operators of an existing towing vessel must have onboard a valid COI prior to the specified phase in period. This vessel has not scheduled an inspection or obtained a valid COI. The vessel shall be made available for a Coast Guard COI inspection immediately and the vessel may no longer be permitted to operate.
