Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. The starboard generator is out-of-service and the port generator failed. Make proper repairs to the generators to prove proper operation to the satisfaction of the attending marine inspector.

Repairs and minor alterations to existing towing vessels must be made in accordance with this part. New installations that are not replacements in kind must comply with the requirements of subpart C of this part. An aux temporary generator was placed on the bow of the vessel. Removal of aux generator prior to departure.

The owner or managing operator of an existing vessel not operating under a stability document must be able to show at least one of the following: The vessel performs successfully on operational tests to determine whether the vessel has adequate stability and handling characteristics. Vessel stability is questioned due to persistent starboard list and excessive stern trim when potable water tanks are filled. Vessel must successfully perform an operational test to the satisfaction of the attending MI to ensure satisfactory vessel stability. A one-time deadship tow transit with assist vessel ITV Raymond (O.N. 997329) from Seadrift, TX City Dock to mile marker 534 of the ICW Homeport of AG Marine Inc. in Ingleside, TX is permitted during daylight hours only, on 30 Jan 2026, in accordance with the submitted transit plan. Any deviation to the plan during the transit shall be reported to the nearest OCMI.

All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Bilge had excessive oily water accumulation of approx. 4-5" in the engine room bilge and the bilge alarm was activated due to oily water level. Removal of oily water from bilge to the satisfaction of the attending MI.

Decks and bulkheads designed to be watertight or weather tight must be maintained in that condition. The hydraulic cable line penetrating through the stern to engine room bulkhead was leaking water into the engine room with the stern void flooded. Temp repairs made. Additionally, There is a hole cut out in the port ceiling weather deck of the engine room. Permanent repairs are to be made to the satisfaction of the attending MI.

Decks and bulkheads designed to be watertight or weather tight must be maintained in that condition. An approximately .5 inch hole was on the stern deck midship of the potable water tank. Permanent repairs are to be made to the satisfaction of the attending MI.

The owner or managing operator must notify the cognizant OCMI when the condition of the vessel, its equipment, systems, or operations, create an unsafe condition. Vessel knowingly operated for an unknown amount of time with multiple unauthorized vessel alterations and unsafe operational/stability criteria and did not report the condition to the OCMI.

Flammable and combustible products must not be stored in machinery spaces, unless they are stored in a suitable container that meets the requirements of § 142.225 of this subchapter. Charcoal bag was stored on the upper deck of the engine room, excessive buckets of oil and oily rags were in the engine room posing a fire hazard. Remove fire hazards to the satisfaction of the attending MI.

Piping and machinery components that exceed 220 °C (428 °F), including fittings, flanges, valves, exhaust manifolds, and turbochargers, must be insulated. Measures must be in place to prevent flammable or combustible liquid piping leaks from coming into contact with these components. Identified missing lagging on exhaust piping of the starboard generator and an approximate 1 ft section of exhaust piping on the port side main engine. Install lagging to satisfaction of MI.

Identified exhaust gas leak coming from lower flange on the starboard main engine exhaust piping. Repair exhaust gas leak to satisfaction of MI.

An exposed hazard such as gears and rotating machinery, must be protected by a cover, guard or rail. The starboard generator was missing guards on the gears and rotating machinery. Provide proper guard to rotating machinery to the satisfaction of the attending MI.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Electrical hazards were observed throughout the engine room. Make proper repairs to the satisfaction of the attending MI.

The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. The starboard stern hatch is missing a dog and pin. Make repairs the the satisfaction of the attending MI.

3-6 inch cracks were identified on 03 blades of the starboard propeller. Due to the extent of the damage, vessel is being asked to repair or replace the damaged propellor.

Each towing vessel must have a fire-detection system installed to detect engine room fires. After loss of ships power, fire detection control panel is inoperable. 46 CFR 142.330(a)

Marine employers must establish programs for the chemical testing for dangerous drugs on a random basis of crew members on inspected vessels who: occupy a position, or perform the duties and functions of a position, required by the vessel's Certificate of Inspection; perform the duties and functions of patrolmen or watchmen required by this chapter; or are specifically assigned the duties of warning, mustering. assembling, assisting, or controlling the movement of passengers during emergencies. Operator does not have a program established or adequate records kept for crew members in accordance with the regulations. 46 CFR 16.230(a)

All fire hoses must be inspected for excessive wear, and subjected to a test pressure equivalent to the maximum service pressure. Port fire hose is leaking at the hydrant coupling. 46 CFR 142.240(a)(7)

Electrical equipment and electrical cables must be suitably protected from wet and corrosive environments. Forward weather deck bulkhead in void below bridge is holed directly above a breaker panel. 46 CFR 143.400 (f)

Alarm set points for the main and auxiliary engine lubricating oil pressure and engine cooling water temperature requiring testing at a frequency of twice every 5 years, with no more than 3 years elapsing since last test. Vessel does not have any records of any such testing being completed. 46 CFR 143.245(b)

Pressure vessel relief valves require testing twice every 5 years with no more than 3 years elapsing since last test. Vessel does not have any records of such testing being completed. 46 CFR 143.245(b)

Each exhaust pipe from an internal combustion engine which is within reach of personnel must be insulated or otherwise guarded to prevent burns. Both main engines, and port generator have exposed high temp piping. 46 CFR 144.830

Means must be provided to ensure that windows immediately forward of the operating station in the pilothouse allow for adequate visibility to ensure safe navigation regardless of weather conditions. Vessel does not have a functioning windshield wiper at the operating station. 46 CFR 144.905(b)

A vessel of 12 meters or more in length shall be provided with a whistle. Captain self-reported that the whistle had stopped working on the vessel on the last voyage.

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. Identified multiple items in the first aid kit that are expired. 46 CFR 140.435

All crewmembers must be provided with health and safety information and training that includes content and procedures of the owner or managing operator's health and safety plan. Crew is not familiar with the vessel’s health and safety plan. 46 CFR 140.515(a)(1)

Each towing vessel must comply with the requirements of part 4 of this chapter for reporting marine casualties and retaining voyage records. Vessel lost power for over 15 minutes while towing and fire detection control panel became inoperative. Crew did not report either to the Coast Guard. 46 CFR 140.900

Decks and bulkheads designed to be watertight or weathertight must be maintained in that condition. Deck containment for bilge water over board is leaking into the E/R. Vessel is authorized a temporary concrete patch to be cropped and renewed to good steel at next DD. 46 CFR 140.610(f)

Each towing vessel must be operated in compliance with: Applicable sections of the Federal Water Pollution Control Act. Vessel was observed with substantial oily water in bilge due to shaft seal leak. Prior to movement the vessel shall remove all oily water from bilge. 140.655(a)

Piping and machinery components that exceed 428 degrees fahrenheit including exhaust piping must be insulated. Properly reattach exhaust manifold lagging to bother main engines. 46 CFR 143.220(b)

An exposed hazard such as gears and rotating machinery must be protected by a cover, guard or rail. Once repairs have been completed to engine shafts ensure machinery guards are in place and properly secured.

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must: Be maintained to ensure proper operation. Vessel was seen in dry dock with both props and starboard shaft pulled. Vessel rep was contacted and reported damage to both props and starboard shaft. Vessel shall enact permanent repairs to the satisfaction of the attending Coast Guard Marine Inspector prior to movement. Prior to putting the vessel back in the water the Coast Guard Marine Inspector must inspect repairs. The vessel shall also conduct a sea trials with the Coast Guard Marine Inspector presence. 46 CFR 143.205 (c)(1)

Towing machinery such as capstans, winches, and other mechanical devices used to connect the towing vessel to the tow must be designed and installed to maximize control of the tow. The forward winches are not installed. Vessel limited to side or stern towing only. Rectify to the satisfaction of USCG marine inspector prior to pushing ahead.

At least two lifebuoys on a towing vessel greater than 26 feet must be fitted with a floating electric water light. The starboard side water light battery was expired and port side water light was missing. Replace to the satisfaction of USCG inspector.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Hot water tank is not secured. Rectify to the satisfaction of USCG marine inspector.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. The electrical enclosure on the Starboard Side of the engine room had a high voltage wire that was not properly secured, the outer insulation was outside of the enclosure showing the insulation of the smaller wires. Rectify to the satisfaction of USCG marine inspector.

Electrical connections must be suitably installed to prevent them from coming loose through vibration or accidental contact. The wires for the generators and main engines batteries are secured using wing nut connections. Rectify to the satisfaction of USCG marine inspector.

The records of inspections and tests of fire-detection systems and fixed fire-extinguishing systems must be recorded in the TVR, official logbook, or in accordance with any TSMS applicable to the vessel. The following minimum information is required: receipts and other records generated by these inspections and tests must be retained for at least 1 year and made available upon request. The servicing information was not available upon request. Rectify to the satisfaction of USCG marine inspector.

A crew accommodation space and a work space must be of sufficient size, adequate construction, and with suitable equipment to provide for the safe operation of the vessel and the protection and accommodation of the crew in a manner practicable for the size, facilities, service, route, and modes of operation of the vessel. The stove top does not have guards/rails to provide adequate protection to crew members and the vessel in all operating conditions. Rectify to the satisfaction of USCG marine inspector.

The owner, manager operator, or master of each towing vessel must maintain the required navigational-safety equipment in a fully-functioning, operational condition. Vessel Anchor light is not operational. Repair or replace to the satisfaction of the attending USCG Marine Inspector.

Has installed, in the engine room and any other area where background noise makes a general alarm hard to hear, a supplemental flashing red light that is identified with a sign that reads: "Attention General Alarm - When Alarm Sounds or Flashes Go to Your Station." GE Alarm is not flashing RED. Repair or replace to the satisfaction of the attending USCG Marine Inspector.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Port generator battery and enclosure are improperly fastened. Secure battery and enclosure to minimize system failure(s) to the satisfaction of the attending USCG Marine Inspector.

Fuel filters must be replaced in accordance with manufacturer's requirements. Port generator fuel filter bottom metal bowl is missing. Repair or replace to the satisfaction of the attending USCG Marine Inspector.

Each towing vessel must be maintained and operated so as to minimize fire hazards. Clean Lube Oil tank in aft section of engine room has a temporary repair on the bottom-right section of the tank. Tank capacity is approximately 80 gallons and is currently filled with 20 gallons of clean lube oil. Vessel rep is to provide a repair proposal to the local OCMI for review and approval.

All owners and operators of an existing towing vessel must have a valid COI prior to the specified phase in period. Owners or managing operators of only one existing towing vessel required to have a COI by this sub chapter must ensure the vessel has on board, a valid COI by July 20, 2020. The vessel shall be made available for a Coast Guard COI inspection within 90 days. If an inspection is not completed prior to this date, the action for this deficiency shall be changed to a 60 (Rectify deficiencies prior to movement) and the vessel may no longer be permitted to operate. Vessel has not yet been able to conduct initial COI. Def code has been changed to a code 60 as 90 days has passed. Vessel is permitted a one time transit from Aransas Marine Ways Shipyard to Live Oak Materials Dock.

Vsl experienced a loss of propulsion due to water in the fuel system.
