The radar display at the primary conning station was not available for inspection.

There was a through bulkhead penetration on the starboard side, just aft of the engine room watertight door that did was not properly blanked off, additionally the forward port port-light for the engine room was sealed with silicone, which is not approved to keep the bulkhead watertight.

When operating a vessel on a body of water where the discharge of treated or untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3 or 140.4, the operator must secure each Type I or Type II device in a manner which prevents discharge of treated or untreated sewage. Acceptable methods of securing the device include - (1) Closing the seacock and removing the handle; (2) Padlocking the seacock in the closed position; (3) Using a non-releasable wire-tie to hold the seacock in the closed position; or (4) Locking the door to the space enclosing the toilets with a padlock or door handle key lock. Missing seacock valve in piping system and means to secure and lock overboard discharge.

Towing vessels not subject to the provisions of 33 CFR part 26 or 33 CFR 164.72 must have a Very High Frequency-Frequency Modulated (VHF-FM) radio installed and capable of monitoring VHF-FM Channels 13 and 16, except when transmitting or receiving traffic on other VHF-FM channels, when participating in a Vessel Traffic Service (VTS), or when monitoring a channel of a VTS. The VHF-FM radio must be installed at each operating station and connected to a functioning battery backup. Neither control station had a VHF radio connected to battery back-up power.
