07114 - A remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. The bracket for the fuel shutoff for the day tank broke when the shutoff was tested. 46 CFR 143.260(b)

09209 - Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. There are exposed wires coming out of the certified safe electrical box next to the fuel transfer system. 46 CFR 143.205

11117 - At least two lifebuoys on a towing vessel greater than 26 feet must be fitted with a floating electric water light approved under approval series 161.010 or 161.110. The water light must be attached by the lanyard with a corrosion-resistant clip to allow the water light to be quickly disconnected from the lifebuoy. The port and starboard waterlights were missing corrosion-resistant clips. The port side waterlight had the incorrect battery which was also expired. 46 CFR 141.360(c)(2) & (3)

07126 - Machinery systems must be maintained to provide for safe operation of the towing vessel and safety of persons onboard. Both generators need to be opened and cleaned to remove excess oil. 46 CFR 143.205(a)

13199 - Machinery systems must be maintained to provide for safe operation of the towing vessel and safety of persons onboard. The jacket water line on the starboard engine was chafed and leaking. 46 CFR 143.205(a)

13199 - If hoses are designed for use with clamps, install with two clamps at each end of the hose. The fuel transfer hose at the transfer station needs a double hose clamp. 46 CFR 146.265(d)(2)(v)(B)

02101 - Decks and bulkheads designed to be watertight or weathertight must be maintained in that condition. The watertight door behind the bridge (containing the day shapes was missing the gasket. 46 CFR 140.610(f)

10112 - A towing vessel must carry adequate and up-to-date charts. The vessel was equipped with 2 systems for electronic charts, but only one system was being updated with up-to-date charts. 46 CFR 140.705 (b)

09218 - Each exhaust pipe from an internal combustion engine which is within reach of personnel must be insulated or otherwise guarded to prevent burns. The exhaust pipe for the port side engine had damaged insulation. 46 CFR 144.830

13101 - Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. The starboard shaft seal had a constant drip. 46 CFR 143.205(a)

11108 - Each liferaft must be arranged to permit it to drop into the water from the deck on which it is stowed. The liferaft was stowed so that it would have to be lifted from the cradle then thrown over the deck to be launched. 46 CFR 141.310 46 CFR 199.130(c)(3)

13101 - Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, be maintained to ensure proper operation. The port and starboard shaft seals packing collars were severely corroded and need to be replaced at the vessel's next drydock (12/31/2024). 46 CFR 143.205(c)

There must be an installed or portable bilge pump for emergency dewatering. Any portable pump must have sufficient hose length and pumping capabilities. All installed bilge piping must have a check/foot valve in each bilge suction that prevents unintended backflooding through bilge piping. The vessel is presently fitted with a fixed bilge system. Conduct operational test of bilge system, including suction(s), from one or more spaces to the satisfaction of the attending Marine Inspectors. UPDATE 5/2/23: The due date has been extended 14 days for this deficiency. The deficiency is now due on 5/19/23.

Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure the following: all bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Excessive standing oil and oily rags observed in engine room bilges.

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquids. Oil leak on port generator that has caused fan/fan shroud, radiator, and surrounding areas to become soaked in oil.

A towing vessel subject to this section must maintain a TVR. The following list of items must be recorded in the TVR: personnel records, safety orientation, records of drills and instruction… The Company’s method of maintaining TVR records is via the Helm program. However, there are no recent TVR records in Helm for most all of the required items (examples: fire drills, safety orientation for new crew, navigation safety assessments, navigation safety training, etc.), and the master of the vessel for the past ~1 month only received Helm log-in/account permissions yesterday.

Paints, coatings, or other flammable or combustible products onboard a towing vessel must be stored in a designated storage room or cabinet when not in use. Along with the primary flammable liquids cabinet of the proper type, there is a second flammable liquids cabinet that is not of a permitted type, currently full of flammable and/or combustible products. Additionally, multiple cans of paint and other flammable or combustible products found stored in the forward storage room.

Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with 33 CFR part 84. Lower masthead light is not functioning.

Means must be provided for closing, in case of a fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces. Vent closure is missing on the starboard side of the stack.

No person may operate any vessel equipped with installed toilet facilities unless it is equipped with an operable Type II or type III device… The MSD’s vent pipe is wasted and no longer connected to the tank, and the level indicator sending unit is not properly wired up.

Pressure vessels over 5 cubic feet (32 gallons) in volume must be equipped with one or more spring-loaded relief valves. The total relieving capacity of the valves must prevent pressure from exceeding the MAWP, as established by the manufacturer, by more than 10 percent. The pressure vessels are labeled as having a 200 psi MAWP, but both relief valves are labeled as having a set-point of 225 psi. The set-point of the relief valves must be 200 psi or less.

The master must ensure that all hatches, doors, and other openings designed to be watertight or weathertight function properly. Two flush-deck scuttles on the main deck aft, port side and center, are not properly and fully seating and sealing.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention : (5) high bilge levels. Alarms must be audible and visual at each operating station. Unable to confirm proper operation of audible and visual bilge alarm on bridge at time of attendance, due to other misc. alarms activated that leave the summary alarm on the bridge panel in a constant alarm status.

The design of the fire detection system and its installation on the towing vessel must be certified and inspected by a registered professional engineer with experience in fire-detection system design, by a NICET Level IV technician, or by an authorized classification society with equivalent experience. There was not a certification letter for the fire detection system that is signed be a PE, NICET Level IV technician, or authorized classification society available onboard at time of attendance.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who occupy a position required by the vessel’s COI. At time of attendance there was no objective evidence available onboard that the Company is meeting the DAPI program requirements. The Company reportedly does not currently use a consortium. A DAPI audit is required to be carried out to the satisfaction of the attending CG personnel.

Each lifebuoy with a floating electric water light must have a lanyard of 3-6 feet in length, securing the water light around the body of the life buoy. Waterlight attachment on starboard life ring not the proper type of line and is tied to the grab line rather than around the life buoy itself.

The inspection will ensure that the vessel is in satisfactory condition and include the structure, piping… Port aft fuel gooseneck vent is wasted and holed.

Piping and machinery components that exceed 428°F, including fittings, flanges, valves, exhaust manifolds, and turbochargers, must be insulated. Lagging on exhaust piping over starboard generator is wasted and falling apart.

Piping and machinery components that exceed 428°F, including fittings, flanges, valves, exhaust manifolds, and turbochargers, must be insulated. Lagging missing over sections of exhaust piping on port generator.

The master must ensure that all hatches, doors, and other openings designed to be watertight or weathertight function properly. Hole in coaming for watertight door from fidley that leads out aft to deck.

A towing vessel that is certificated for lakes, lays, and sounds must be equipped with either an approved 160B semi-portable extinguisher or a fixed fire-extinguishing system installed to protect the engine room. A fixed fire-extinguishing system means a carbon dioxide system that meets the requirements of 46 CFR subpart 76.15, 79.47-9, 78.47-11, and that is approved by the Commandant. 46 CFR 76.15-20 (c) requires that the cylinders be securely fastened and supported, and 76.15-20 (b) requires that if the cylinders are located within the space protected, the system shall be arranged in an approved manner to be automatically operate by a heat actuator within the space in addition to the regular remote and local controls. The vessel is fitted with a fixed CO2 system consisting of 4 x 75 lb bottles of agent. The bottles are installed inside the space but not arranged for automatic operation by a heat actuator. In addition, they are not properly fastened/supported. Update 4/5/23: the deficiency due date is extended until 9/1/23. The securing/supporting of the bottles has been corrected as of this date.

Each towing vessel must perform the applicable tests in Table 143.245 (b) of this section. The tests required by this section must be recorded in accordance with part 140 of this subchapter. Alarm setpoints: twice every 5 years, with no more than 3 years elapsing since last test. (see 143.230 (a) for list of required alarms). At time of attendance, the vessel has held a COI for >3 years, but there is no objective evidence of completion of testing of the alarm setpoints.

Machinery must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid into the machinery space. Leak on hydraulic system piping near port aft side of engine room, with hanging catch can about ½ full of oil.

The owner or managing operator must implement a health and safety plan. All crewmembers must be provided with health and safety information and training that includes: content and procedures of the owner or managing operator’s health and safety plan… The current Health and Safety Plan has multiple inaccuracies. For example, the provided company organization chart lists people that are no longer with the company, and the plan states that the company’s vessels comply with the line throwing device requirements, but no linethrowing devices are available onboard (or required based on the vessel’s route). Additionally, there is no objective evidence that crewmembers have received training on the Health and Safety Plan as required.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Emergency lighting must be provided for all internal crew working and living areas. E-light in engine room between the two generators is not properly mounted and just hanging by the wire nut connections.

Each towing vessel must be equipped with navigation lights in accordance with 33 CFR part 84. Vessel is not presently equipped with Not Under Command and Restricted in Ability to Maneuver Lights.

Each nonmetallic transfer hose must be tested at 1 ½ times the maximum allowable working pressure annually. There is no objective evidence of annual hydrostatic test records for fuel transfer hose mounted on hose reel. *Must be corrected prior to using this hose for fuel transfer.

The operator of a vessel with a capacity of 250 or more barrels of oil as regulated in Table 4 of 46 CFR part 154 shall provide transfer procedures that meet the requirements of this part and part 156 of this chapter for transferring - (a) To or from the vessel; and (b) From tank to tank within the vessel. Transfer procedures inadequately address their applicability to transfers within the vessel between tanks. The lack of adequate procedures resulted in an unsupervised transfer and subsequent discharge of diesel fuel into the water. 33 CFR 155.720

At least two lifebuoys on towing vessels greater than 26 ft. must be fitted with a floating electric water light approved under 161.010 or 161.110 unless the towing vessel is limited to daytime operation. The water light installed on the starboard side was not working at the time of inspection. Vessel is restricted to daytime operations only until waterlight is replaced.

Vessel's EPIRB is currently installed with an expired hydrostatic release and the registration is expired. Vessel's route does not currently require an EPIRB but if the owner/operator chooses to have one on board it must be maintained properly.

Each lifejacket must be marked with the vessel's name in block capitol letters. Two lifejackets had faded or illegible writing and must be relabeled.

Pressure vessel relief valves must be set to a total relieving capacity that prevents pressure from exceeding MAWP by more than 10%. The pressure relief valves on two air receivers were set at over 300 PSI with tank MAWP marked at 200 PSI. Replace with compliant relief valves.

Piping and machinery components that exceed 220 degrees Celsius (428 F), including fittings, flanges, valves, exhaust manifolds, and turbochargers must be insulated. Engines were missing lagging where the exhaust/scavenger air uptake meet at the top of the engine. Replace lagging.

Electrical components that pose an electrical hazard must be in an enclosure. Both MDE gauge panels are missing the back coverings, exposing wires. Secure all wiring,replace missing gauges & replace back panels.

The following systems must be equipped with gauges at the machinery location: (2) Main engine high cooling water temperature. Vessel's machinery gauge display panel was missing main engine high cooling water temperature gauges for both port and starboard main engines. Install gauges for each engine.

All seals and gaskets must be properly maintained to prevent leaks. Detroit diesel generator is displaying a slow oil leak at the bellows joint. Replace gaskets and rectify leak by next dry dock (DEC 2022).

Provisions shall be made to maintain the watertight and weathertight integrity of the vessel. A small hole was observed in the port side combing below the hatch leading into the upper engine room space. Additionally, the starboard propeller shaft is leaking at the through-hull penetration. Repair by next dry dock (DEC 2022).

Paints, coatings, or other flammable or combustible products onboard must be stored in a designated storage room or cabinet when not in use. Vessel had approximately 05 cans (3-5 gallons each) of gasoline that was being stored in the vicinity of a flammable cabinet located on the exterior of 2nd deck, aft of the pilot house, but there was not sufficient space inside the cabinet for them to be stowed. Vessel's crew removed the gasoline cans from the vessel at the conclusion of the inspection and plan to order a second cabinet for increased stowage capacity in the future.

Vessel #2 generator is inoperable. Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Vessel must replace generator in kind or provide 3rd party service report that generator has been appropriately fixed. 46 CFR 143.400 (a)

No guard present on starboard side engine rotating machinery.

Inoperable low lube oil alarm for starboard side engine.

Battery charger not permanently attached with alligator clips.

Fuel shut off valve is not properly labeled.

Heat sensor in the galley was damaged during testing.
