On 27 FEB 96 while transferring fuel to the M/V TOM G the person in charge didn't load the vessel in accordance with the vessels transfer procedures. The procedures stated that the tanks were to be loaded to a maximum of 10" below deck level. The tanks were loaded to within 4" of the deck.
