Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Both Port and STBD generator had excessive oil and grease build up in the electrical generation component (stinger) of the gensets. Perform routine maintenance/cleaning to prevent fire hazard.

Prior to getting underway for the first time on a particular towing vessel, each crewmember must receive a safety orientation. There was no record of safety orientation onboard. Ensure crew is properly trained on the vessel and their responsibilities. Provide copies of safety orientation to local OCMI (MSU Pittsburgh).

The Master of a towing vessel must ensure that each crewmember participates in fire-fighting drills and receives instruction at least once each month. There was no record of any fire drill onboard. Once the vessel is crewed conduct a fire drill and provide a copy of the record to local OCMI (MSU Pittsburgh).

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid, as those terms are defined in 46 CFR subpart 30.10, into the machinery space. Starboard engine turbocharger lube oil drain line leaking oil(2-3 per second). Repair oil leak as required.

The TSMS, through policies, procedures, and documentation, must: (a) Demonstrate management responsibility. The management must demonstrate that they implemented the policies and procedures as contained in the TSMS and the entire organization is adhering to their safety management program. (b) Document management procedures. A TSMS must describe and document the owner or managing operator's organizational structure, responsibilities, procedures, and resources which ensure quality monitoring. (c) Ensure document and data control. There must be clear identification of what types of documents and data are to be controlled, and who is responsible for controlling activities, including approval, issue, distribution, modification, removal of obsolete materials, and other related administrative functions. (d) Provide a process and criteria for selection of third parties. Procedures for selection of TPOs must exist that include how third parties are evaluated, including selection criteria. (e) Establish a system of recordkeeping. Records must be maintained to demonstrate effective implementation of the TSMS. This must include audit records, non-conformity reports and corrective actions, auditor qualifications, auditor training, and other records as considered necessary. (f) Identify and meet training needs. The owner or operator must establish and maintain documented procedures for identifying training needs and providing training. (g) Ensure adequate resources. Identify adequate resources and procedures necessary to comply with the TSMS. Vessel master, crew, and management found to be unfamiliar/not in compliance with Towing Safety Management System (TSMS). Vessel is required to undergo External survey and audit demonstrating understanding and compliance with vessel TSMS to the satisfaction of the TPO.

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. Smoke detector in Masters and Pilots birthing room disabled (Battery removed). Install working smoke detector and conduct training to ensure crew is not disabling fire detection systems.

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces. Ventilation louvers found to disabled by wooden blocks and damaged. Install working louvers and conduct training with crew to ensure fire systems are not disabled.

A fixed fire pump must be capable of delivering water simultaneously from the two highest hydrants, or from both branches of the fitting if the highest hydrant has a Siamese fitting, at a pitot-tube pressure of at least 344 kilopascals (kPa) (50 pounds per square inch (psi)), and a flow rate of at least 300 liters per minute (lpm) (80 gallons per minute (gpm)). Fire pump not producing adequate water. Repair or provide portable fire pump.

All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Station boarding found engine room bilges to have 2000 gallons of oily water mixture. Pump out and clean bilges as required.

To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. Remote fuel shut off valves inoperable when operating from outside the engine room. Repair as required.

Portable and semi-portable fire extinguishers must be inspected, maintained, and tested in accordance with the inspection, maintenance procedures, and hydrostatic pressure tests required by Chapters 7 and 8 of NFPA 10, Standard for Portable Fire Extinguishers (incorporated by reference, see § 136.112 of this subchapter), with the frequency specified by NFPA 10. Annual inspection of portable fire extinguisher by MSD not completed and mounting bracket found broken. Have extinguisher inspected as required

Each towing vessel must be operated in compliance with: (1) Applicable sections of the Federal Water Pollution Control Act, including section 311 of the Federal Water Pollution Control Act, as amended (33 U.S.C. 1321); (2) Applicable sections of the Act to Prevent Pollution from Ships (33 U.S.C. 1901 et seq.); and (3) Parts 151, 155, and 156, of 33 CFR, as applicable. Bilge well at port shaft packing found with oil and grease inside. Clean bilge well to prevent pollution.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Repair the following deficiencies: 1. Loose batteries installed for electronics under bridge 2. Loose battery connection on port generator 3. Outside electrical receptacle missing cover 4. Aft deck lights electrical cable splices not inside an enclosure 5. Aft birthing room light fixture missing dome and has exposed electrical light fitting

Each towing vessel must be fitted with a general alarm that has installed, in the engine room and any other area where background noise makes a general alarm hard to hear, a supplemental flashing red light that is identified with a sign that reads: “Attention General Alarm—When Alarm Sounds or Flashes Go to Your Station”. Engine room general alarm light dim and does not provide suficient light to be seen throughout the engine room. Install ight with visibility.

Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with the International Regulations for Prevention of Collisions at Sea (COLREGS) or 33 CFR part 84 as appropriate to its area of operation. Mast head light mounting wasted. Replace with new.

All navigation lights found to have household light bulbs installed. Install manufacture approved bulbs that meet the visibility requirements set in 33 CFR 83.22.

The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. Past navigation assessments found with missing information/incomplete. Conduct training with crew to ensure navigation assessments are properly completed.

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. Eyewash expired. Replace with new.

Rails or equivalent protection must be installed near the periphery of all decks accessible to crew. Equivalent protection may include lifelines, wire rope, chains, and bulwarks that provide strength and support equivalent to fixed rails. Rails or equivalent missing on bow. Install chain or railing as required.

Paints, coatings, or other flammable or combustible products onboard a towing vessel must be stored in a designated storage room or cabinet when not in use. 5 gallon bucket of engine oil stored under engine room stairs. Remove oil and store in proper location.

A towing vessel must carry adequate and up-to-date charts, maps, and nautical publications for the intended voyage. Vessel's navigation rules and light list were out of date. Update publications as required.

Each watertight hatch must be marked in clearly legible letters at least 25 millimeters (1 inch) high: “WATERTIGHT HATCH—KEEP CLOSED”. Watertight hatches not properly labeled. Label as required.

Pressure vessels must be externally examined annually. Relief valves must be tested in accordance with § 143.245 (Twice in 5 not to exceed 3). Inspect pressure relief valves as

Piping and machinery components that exceed 220 °C (428 °F), including fittings, flanges, valves, exhaust manifolds, and turbochargers, must be insulated. Measures must be in place to prevent flammable or combustible liquid piping leaks from coming into contact with these components. Both port and starboard engines missing exhaust lagging on turbo charges. Install lagging as required.

Individual circuit breakers on switchboards and distribution panels must be labeled with a description of the loads they serve. 220V breaker panel in engine room missing label. Label loads as required.

Each lifejacket must have a whistle firmly secured by a cord to the lifejacket. Life jacket in aft birthing missing whistle. Install new whistle.

Steering hydraulic system failed, causing a loss of propulsion. Repair as required to ensure hydraulic system is free of debris and operates as designed.
