At least two lifebuoys on a towing vessel greater than 26 feet must be fitted with a floating electric water light approved under approval series 161.010 or 161.110. Both lifebuoys were outfitted with float lights attached to spring type connectors of non-approved batteries. The proper screw post type batteries, as approved on the light, shall be installed.

The TSMS, through policies, procedures, and documentation, must: (a) Demonstrate management responsibility. The management must demonstrate that they implemented the policies and procedures as contained in the TSMS and the entire organization is adhering to their safety management program. (b) Document management procedures. A TSMS must describe and document the owner or managing operator's organizational structure, responsibilities, procedures, and resources which ensure quality monitoring. (c) Ensure document and data control. There must be clear identification of what types of documents and data are to be controlled, and who is responsible for controlling activities, including approval, issue, distribution, modification, removal of obsolete materials, and other related administrative functions. (d) Provide a process and criteria for selection of third parties. Procedures for selection of TPOs must exist that include how third parties are evaluated, including selection criteria. d) Provide a process and criteria for selection of third parties. Records must be maintained to demonstrate effective implementation of the TSMS. This must include audit records, non-conformity reports and corrective actions, auditor qualifications, auditor training, and other records as considered necessary. (f) Identify and meet training needs. The owner or operator must establish and maintain documented procedures for identifying training needs and providing training. (g) Ensure adequate resources. Identify adequate resources and procedures necessary to comply with the TSMS. Vessel master, crew, and management found to be unfamiliar/not in compliance with Towing Safety Management System (TSMS). Vessel is required to undergo External survey and audit demonstrating understanding and compliance with vessel TSMS to the satisfaction of the TPO. Vessel shall capture the following deficiencies IAW it's TSMS and to the satisfaction of the Coast Guard.

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must: (1) Be maintained to ensure proper operation. Starboard inboard flanking rudder post found recessed 8"-10" into rudder yoke. Repair so rudder post extends to top of yoke as designed.

The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. Starboard aft watertight hatch and ring in engine room wasted and unserviceable. Replace hatch assembly with new.

All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Engine room bilge found with oil soaked rags, oil debris, and approximately 4"-5" of oil in bilge. Remove rags, debris, and oil as required.

Each towing vessel must perform the applicable tests in Table 143.245(b) of this section. The tests required by this section must be recorded in accordance with part 140 of this subchapter (pressure relief valves - twice every 5 years, with no more than 3 years elapsing since last test). Start air pressure vessels not tested in over 3 years. Test or replace.

Each lifejacket must be marked: (1) In block capital letters with the name of the vessel; and (2) With Type I retro-reflective material approved under approval series 164.018. The arrangement of the retro-reflective material must meet IMO Resolution A.658(16) (incorporated by reference, see § 136.112 of this subchapter). Vessel Missing 01 lifejackets and 01 lifejackets retro unserviceable. Repair or replace as required.

Stowage positions for lifejackets stowed in a berthing space or stateroom and all lifejacket containers must be marked in block capital letters and numbers with the minimum quantity, identity, and, if sizes other than adult or universal sizes are used on the vessel, the size of the lifejackets stowed inside the container. The equipment may be identified in words or with the appropriate symbol from IMO Resolution A.760(18) (incorporated by reference, see § 136.112 of this subchapter). Lifejackets stored in berthing p-way closet and in pilothouse without label. Label as required.

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces. No means available for engine room ventilators in case of a fire. Install a method to close all engine room ventilation.

Inspection and testing. All portable fire extinguishers, semi-portable fire-extinguishing systems, fire-detection systems, and fixed fire-extinguishing systems, including ventilation, machinery shutdowns, and fixed fire-extinguishing system pressure-operated dampers on board the vessel, must be inspected or tested at least once every 12 months, as prescribed in paragraphs (a)(1) through (8) of this section, or more frequently if otherwise required by the TSMS applicable to the vessel. Vessel Semi-portable extinguisher annual service documentation unavailable. Provide documentation or service as required.

Each remote valve control must be marked in clearly legible letters, at least 25 millimeters (1 inch) high, indicating the purpose of the valve and the way to operate it. Fuel cutoffs missing directions.

Smoke detection in berthing spaces. Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. This may be accomplished by an installed detection system, or by using individual battery-operated detectors meeting UL 217 (incorporated by reference, see § 136.112 of this subchapter). Detector in port side aft berthing space inoperable. repair as required.

Rails or equivalent protection must be installed near the periphery of all decks accessible to crew. Equivalent protection may include lifelines, wire rope, chains, and bulwarks that provide strength and support equivalent to fixed rails. Forward maindeck and tow knees missing fall protection around periphery. install as required.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Nylock nut and bolt in steering linkage located in port berthing closet not engaging nylock.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. The following deficiencies were identified: • Outlet covers in galley broken /missing. Replace covers with new • Wire to port spotlight-exposed at unmounted j-box. Permanently mount j-box and cover exposed wire • Wire to MSD power cable exposed at plug. cover exposed wire • Dead end wires found in lower level engine room overhead. Remove dead end wire • Lower level engine room light cover missing. Install new light cover • No GFI receptacle or breakers found near sources of water in galley, engine room, or heads. Install GFI receptacles or breakers • Nonconductive matting installed in front of electrical switchboard for shore power/generator. Install nonconductive matting in front of switchboard • Lockout for generator / shore power does not prevent the simultaneous activation of both shore and #2 generator. Install lock-out that prevents simultaneous activation of power sources.

A towing vessel must carry a combination of day and night visual distress signals indicated in Table 141.370 of § 141.370 for specified areas where the vessel operates. Insufficient number of day flares on vessel and all flares are expired. Replace flares with new.

Each watertight door and watertight hatch must be marked on both sides in clearly legible letters at least 25 millimeters (1 inch) high: “WATERTIGHT DOOR - KEEP CLOSED” or “WATERTIGHT HATCH - KEEP CLOSED”. Watertight hatches not properly labeled. Label hatches as required.

A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. Approximately 1’ of water found in forward rake. Pump out water.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Port engine replaced in kind due to reported metallic noise. Complete installation and conduct operational test.

Except as provided by paragraph (b) of this section, each navigation light must - (1) Meet the technical standards of the applicable Navigation Rules; (2) Be certified by a laboratory listed by the Coast Guard to the standards of ABYC A-16 (incorporated by reference, see § 183.5) or equivalent, although portable battery-powered lights need only meet the requirements of the standard applicable to them. Stern light lens cover contaminated and not allowing appropriate light transmission.
