Permanently repair or replace the affected section of piping and test the repair to the satisfaction of the third-party organization (TPO). Upon satisfactory completion, provide this office with a report from the TPO documenting the repair and test results.

46 CFR 143.605 / 10c - During the vessel's transit to Port Angeles, Wa, vessel experienced a loss of propulsion as a result of overspeed alarms and shutdowns of both main propulsion engines. It was determined that the propulsion machinery operated as designed and that both main engines restarted following normal procedures. An official statement by the Chief Engineer and copies of the engineering and brdige log book were required to verify that all systems operated as designed. Vessel cleared to sail.
