Prior to getting underway, and at all other times necessary to ensure the safety of the vessel, the master or officer in charge of a navigational watch must determine whether the vessel complies with all stability requirements in the vessel's trim and stability book, stability letter, COI, and Load Line Certificate, as applicable. A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. Also, All towing vessels must be operated in a manner that minimizes the risk of down-flooding and progressive flooding. The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. The gasket seals on the following were found not keeping a watertight seal, with daylight visible from inside: 1. Rope and paint locker hatch, 2. port and starboard accommodation door hatches. Towing vessel shall ensure watertightness. 46 CFR 140.605 (a) & (b) 46 CFR 140.610 (a), (b) & (c)

All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Vessels engine room bilge had excess oil accumulation. Remove excess oil. 46CFR144.220(a)

Vessels with a flammable storage locker must have a 40-B portable fire extinguisher located near the storage room or cabinet. Vessel's portable fire extinguisher located at the flammable storage locker was rusted and seized. 46CFR144.225(d)

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. Vessels bilge alarm wiring was disconnected, repair. 46CFR143.230(a)

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Vessels alcohol swabs were expired. Replace alcohol swabs. 46CFR4.06-15(a) (1)

All owners and operators of an existing towing vessel must have a valid COI on board no later than July 19, 2022. This vessel has not obtained a valid COI and is no longer permitted to operate. Vessel must complete certification at first U.S. Port.

Replace non marine grade connectors for marine connectors on the bridge light and console or tape them in place with electrical tape.
