When conducting a survey of the vessel, the surveyor must determine that the item or system functions as designed is free from defects or modifications that reduce it effectiveness, is suitable for the service and functions safely in a manner consistent or vessel type, service and route. The steering cylinder foundation's perimeter weld is cracked and the deck plating that the foundation is welded too is also cracked. When the steering cylinder is actuated, the cylinder's foundation lifts off the deck and the deck plating flexes. Conduct Internal Structural Exam of the aft void to determine the condition any affected framing. Complete repairs in accordance with NVIC 7-68 and to the satisfaction of the TPO.

The master must ensure that all hatches, doors and other openings designed to be watertight function properly. The watertight door in the aft engine room bulkhead does not have a gasket installed. Install gasket and conduct chalk test to ensure that door is capable being made watertight.

A towing vessel must carry adequate and up-to-date charts, maps, and nautical publications for the intended voyage, including: (1) Charts, including electronic charts acceptable to the Coast Guard, of appropriate scale to make safe navigation possible. Vessel was found operating with electronic charting system that does not have a type approval or manufacturer’s declaration of conformity and therefore does not meet the chart equivalency requirement.
