All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. MI observed an excess amount of oily water in the bilge. 46 CFR 142.220(a)

Each towing vessel must be fitted with a general alarm that: Is activated at each operating station and can notify persons onboard in the event of an emergency. MI observed the emergency alarm inoperable during a loss of power event. 46 CFR 143.235(b)(1)

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid into the machinery space. MI observed a hydraulic oil leak on the steering pump service tank. 46 CFR 143.220(a)

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. MI observed an excessive raw water leak on the port main engine. 46 CFR 143.205(a)

The following systems must be equipped with gauges at the machinery location: main engine RPM. MI observed the starboard main engine RPM gage inoperable.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. MI observed a persistent exhaust leak from the port main engine turbo seal. 46 CFR 143.205(a)

Piping and machinery components that exceed 220 °C (428 °F), including fittings, flanges, valves, exhaust manifolds, and turbochargers, must be insulated. MI observed insufficient exhaust lagging on the starboard generator. 46 CFR 143.220(b)

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. MI observed all spare breakers left in the "on" position. 46 CFR 143.400(a)

All owners & operators of an existing towing vessel must have a valid COI prior to the specified phase-in period. This vessel is on of the 25% of the vessels that has not obtained a valid COI. the vessel shall be made available for a Coast Guard COI inspection within 90 days*. If an inspection is not completed prior to this date, the action for this deficiency shall be changes to a CODE 60(Rectify deficiencies prior to movement) and the vessel may no longer be permitted to operate. *Your vessel is utilizing the TPO/TSMS compliance option, therefore your vessel shall meet all the COI requirements of your TPO and submit all required documentation to our office in Memphis, TN.
