The fire main system must be charged, and sufficient pressure must be verified at the most remote and highest outlets. Excessive amounts of water was observed to be leaking from the fire pump shaft, pressure side.

Portable and semi-portable fire extinguishers must be inspected, maintained, and tested in accordance with the inspection, maintenance procedures, and hydrostatic pressure tests required by Chapters 7 and 8 of NFPA 10, Standard for Portable Fire Extinguishers (incorporated by reference, see § 136.112 of this subchapter), with the frequency specified by NFPA 10. No servicing records were available proving annual servicing of the semi-portable fire extinguisher.

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces. Machinery space ventilation damper on starboard side found with spring closure device disconnected.

All lifesaving equipment must be tested and maintained in accordance with the minimum requirements of § 199.190 of this chapter, as applicable, and the vessel's TSMS, if the vessel has a TSMS. The starboard rigid life raft has expired seasickness medication and expired water packages and floating ring life buoy light has expired battery.

Lifejackets must be readily accessible. The lower AB state room was missing (01) lifejacket and the ENG stateroom was missing (02) lifejackets.

All fire-detection equipment and systems on board a towing vessel must be maintained in accordance with the attached nameplate and manufacturer's approved design manual. The fire detection system is inoperable; the alarm panel is displaying “system trouble” and “PSU trouble” alarms.

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must: Be maintained to ensure proper operation. An exhaust leak was observed on the port and starboard generator.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Aft “house” generator found with excessive oil build up and rear leaking main seal. **26MAY26** changed action to 705 - prior to use as vsl crew stated they do not use during operating season and plan to renew house generator

Repairs and minor alterations to existing towing vessels must be made in accordance with this part. Remove sump pump installed in the “decanting” tank.

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid, as those terms are defined in 46 CFR subpart 30.10, into the machinery space. Day tank stripping valve found leaking fuel.

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must: Be maintained to ensure proper operation. Port main engine found with unsecured lagging on turbo.

Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. The aft starboard ballast tank goose neck vent guard found unsecured.

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. First aid kit found with expired medication and eyewash station in engine room found with unknown last date of service.

Electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Dead end wires were found near port shaft on aft engine room bulkhead.

Except as provided in paragraph (d) or (e) of this section, a towing vessel with overnight accommodations and alternating watches (shift work), when pulling, pushing or hauling alongside one or more barges, must have a system to detect when its master or mate (pilot) becomes incapacitated. The vessel does not have a pilothouse alerter system installed onboard.

Each watertight door and watertight hatch must be marked on both sides in clearly legible letters at least 25 millimeters (1 inch) high: “WATERTIGHT DOOR—KEEP CLOSED” or “WATERTIGHT HATCH—KEEP CLOSED”. The watertight doors on the main deck and the watertight hatch were not labeled.

Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space. Engine room emergency lighting did not light emergency escape path.

In each crew berthing area, sexual assault and sexual harassment information, as required by 46USC11101, must be posted. Required information was not posted onboard. 46 USC 11101 (MISLE not updated with applicable code)

Chemical testing of personnel must be conducted as required by this subpart and in accordance with the procedures detailed in 49 CFR part 40. No records could be provided of crew enrollment or testing.

Where practicable, each space where crew may be quartered or normally employed must have at least two means of escape. The forward emergency escape ladder on the starboard side was blocked.

Each towing vessel must be equipped with shapes in accordance with the 33CFR84. Day shapes for anchoring were missing.

Records, unless required to be maintained for a longer period by statute or other federal regulation, must be retained for at least 1 year after the date of the latest entry. The crew was unable to provide Towing Vessel Record entries for various items to be recorded under 46 CFR 140.915, such as navigation assessments, oil residue discharges, inspection of towing gear, etc.

A 40-B portable fire extinguisher must be located near the storage room or cabinet. This is in addition to the the portable fire extinguishers required by tables 142.230(a) and 142.230(b) of this part. Portable 40-B extinguishers missing near engine room flammable locker and (02) deck flammable locker.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. Main engine, generator, and hydraulic steering gear system alarms were not operable. **26MAY26** updated action to 50 as the vessel has to order alarm

Each towing vessel must perform the applicable tests in Table 143.245(b) of this section. Pressure vessel relief valves were unable to be tested by ship’s crew.

Each towing vessel must be operated in compliance with Parts 151, 155, and 156, of 33 CFR, as applicable. Port side small discharge containment missing plug.

It is the responsibility of the crew to comply with the applicable provisions of this subchapter. Crew was unfamiliar with operation and testing of remote fuel shutoff valve.

A 40-B portable fire extinguisher must be located near the storage room or cabinet. This is in addition to the portable fire extinguishers required by tables 142.230(a) and 142.230(b) of this part. Portable 40-B extinguishers missing near engine room flammable locker and (02) deck flammable locker.

The general alarm was not functional on e-power nor ship's power. Ensure general alarm is operational to include audibly and visually.

Non of the high water alarms in the laz were operational. Ensure all high water alarms in the laz are operational.

Ensure the first aid chest is up-to-date.

Ensure the vessel is equipped with a pilothouse alerter system prior to the completion of next scheduled drydock.

The design of the system and its installation on the towing vessel is certified and inspected by a registered professional engineer with experience in fire-detection system design, by a technician with qualifications as a National Institute for Certification in Engineering Technologies (NICET) level IV fire alarm engineering technician, or by an authorized classification society with equivalent experience, to comply with paragraphs (a)(1) through (6) of this section. The vsl installed a new fire detection panel. Provide proof that the system was installed by an approved person.

Aft port fuel tank - pin hole in aft bulkhead at bottom long, crop and renew.

After any survey of the vessel made under §§ 42.09-25 to 42.09-40, inclusive, as applicable, has been completed and deficiencies corrected as provided in § 42.09-45, no change shall be made in the vessel's structure, equipment, arrangement, material, or scantlings as covered by such survey, without the prior specific approval of the assigning and issuing authority. A crack was discovered on the port side tank bulkhead at the bottom weld seam of the #5 Port Wing Tank and the hull plating located below the waterline. Weld repairs were made by non-certified ABS welders and no notification was made to the assigning and issuing authority (ABS). Notify ABS of the unauthorized weld repairs and provide permanent repair proposal to satisfaction of attending Marine Inspector. 05SEP2024: Received repair proposal to conduct permanent repairs in Seward, AK. Vessel rep to schedule CG attendance to verify final repairs.

Ensure E-light next to ER ladder is operational.

Winter plug is missing from bilge pumps. Install plugs.

Ensure laz high water alarm is operational.

Perform tests as required by table 143.245(b). Tests must be logged IAW Part 140.

Ensure the vessel has a valid VGP. PL 11-01

Remove or place in junction box dead end wire in the lower port stateroom.

Ensure first aid kit is up-to-date.

Provide blank for open area in engine room switchboard

Provide oil transfer procedures.

Ensure Type II MSD is operated IAW MSD Manual.

Ensure all crew that serve in a safety sensitive position is enrooled in a CG approved drug testing program. Vessel had 3 crew enrolled and may operate for 12 hrs or less in any 24 hour period on a voyage of less than 600 NM. Crew must be master and 2 ABs.

Vessel had not conducted any drills. Conduct abandon ship, MOB, fire, and flooding drills.

Lazarette high water alarm was inop. Repair or renew.

Remote fuel shut off valve was not operational. Repair or renew.

Six immersion suits did not have lights attached or were not operational. Ensure all immersion suits have approved lights. Vessel has more than needed properly outfitted immersion suits onboard.

Two E-lights were extinguished on the main deck. Repair or renew.

Ensure all general alarms are labeled properly.
