Issued 26 February 2024 Resolved
All vessel equipment must be used in accordance with the manufacturers recommended practice and in a manner that minimizes risk of injury or death. The marine sanitation device (MSD) venting pipe is configured in such away that the MSD tank is venting into the generator machinery space. Current configuration allows toxic and flammable gases such as Hydrogen Sulfide and Methane gas to be vented into the space. Additionally, there to suggest that sewage has overflowed from the MSD vent into the bilge. MI's found approximately 2 inches of liquid substance in the bilge that is believed to be sewage. MI's also found dried toilet paper on deck framing underneath the MSD tank. The generator space bilge is required to be pumped out, cleaned and sanitized. MSD vent pipe must be reconfigured to allow the MSD system to vent outside the vessel and into the atmosphere.

An exposed hazard such as gears and rotating machinery, must be protected by a cover, guard or rail. There is no guards or rails preventing personnel from contacting the shaft brake rotor assembly of both the port and starboard propulsion shafts. Install appropiate guard or rail to protect personnel from injury.
