Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Lifeboat window bolts were overtightened cracking windows in several spots. Additionally, seal between lower hull and topside was deteriorated across the stern 74SOLAS (20) III/20.2

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship on all respects will remain fit to proceed to sea without danger to the ship or persons on board. The cargo pilot line on the # 3 Port Framo pump ruptured during discharge operations. MI observed the crew activate ESD during an actual emergency. Vessel must secure the # 3 Port pump remote activation hydraulic line and blank-off the connection at the pump. 74 SOLAS (14) I/ 11(a) 705 (A)

When a distribution system, whether primary or secondary, for power, heating or lighting, with no connection to earth is used, a device capable of continuously monitoring the insulation level to earth and of giving an audible or visual indication of abnormal low insulation values shall be provided. At the time of the inspection, it was noted that a partial ground was found on the "B" phase of the 440VAC distribution panel. SOLAS09 II-I/Reg.45.4.2

Ships shall be provided with arrangements, equipment and fittings of sufficient safe working load to enable the safe conduct of all towing and mooring operations associated with the normal operation of the ship. The crew reported a punctured hydraulic pipe used for mooring equipment on the main deck which was observed leaking hydraulic oil when under pressure. The piping is not in compliance with the classification society standards and poses a potential pollution threat. A temporary steel doubler was welded over the punctured piping until the pipe can be replaced in kind. SOLAS 2009 Cons. II-1 3-8.2

IN ADDITION TO THE REQUIREMENTS CONTAINED ELSEWHERE IN THE PRESENT REGULATIONS, SHIPS SHALL BE DESIGNED, CONSTRUCTED AND MAINTAINED IN COMPLIANCE WITH THE STRUCTURAL, MECHANICAL AND ELECTRICAL REQUIREMENTS OF A CLASSIFICATION SOCIETY WHICH IS RECOGNIZED BY THE ADMINISTRATION IN ACCORDANCE WITH THE PROVISIONS OF REGULATION XI-1/1, OR WHITH APPLICABLE NATIONAL STANDARDS OF THE ADMINISTRATION WHICH PROVIDE AN EQUIVALENT LEVEL OF SAFETY. VESSEL MAIN ENGINE IS UNABLE TO BE RUN IN REVERSE. PRIOR TO DEPARTURE FROM ANCHORAGE, PROVIDE CLASS REPORT ATTESTING TO THE PROPER OPERATION OF THE MAIN ENGINE.
