Each individual bilge suction shall be fitted with a suitable bilge strainer having an open area of not less than 3 times that of the suction pipe. All bilge suctions had strainers removed. Replace and provide documentation attesting to installation.

For industrial vessels of 300 gross tons and over but less than 4,000 gross tons, contracted for prior to July 1, 1968, which carry in excess of 12 industrial personnel, existing structure arrangements and materials previously approved will be considered satisfactory so long as they are maintained in good condition to the satisfaction of the Officer in Charge, Marine Inspection. MI found structural wastage, openings, and holes along the entire machinery space boundary, a space protected by CO2 fixed extinguishing system. 46 CFR 92.07-90(b), 46 CFR 95.15-35(b)(c), Action Code 705: Until current contracted work is completed in Southwest Pass.

The inspection shall be such as to insure that the vessel, as regards the structure is in satisfactory condition and fit for the service for which it is intended. During the general walk-through MI discovered multiple holes in the superstructure of the vessel. Concerning holes and wastage was found on bulkhead to the machinery space, fuel vents, decking, ladder wells and stanchions supporting the 03 Portable Accommodation Module. Complete repairs to the satisfaction of the attending marine inspector at the next dry-dock, anticipated in OCT 2024. 46 CFR 91.25-10, Action Code: 705

General arrangement plans showing for each deck the fire control stations, the various sections enclosed by fire-resisting bulkheads. When reviewing the fire control plan. Plan did not have markings for fire-resisting bulkheads. Revise plan to the satisfaction of the attending Marine Inspector at the next dry-dock, anticipated in OCT 2024. 46 CFR 97.36-1(a), Action Code: 705, Complete at dry-dock

Each power bilge pump must have the capacity to develop a suction velocity of no less than 400' per minute. Vessel was not able to draw suction from bilge pump in engineer plant.

Fire pumps may be used for other purposes provided at least one of the required pumps is kept available for use on the fire system at all times. The #2 fire pump has excessive water leaking from the packing. Repair to the satisfaction of attending MI.

Fire Main - All piping, valves and fittings shall meet subchapter F. Vessel had at least 02 soft patches applied to it. Renew piping to the satisfaction of the attending MI.

In spaces where the general emergency alarm signal cannot be heard over the background noise, there must be a red flashing light or rotating beacon. Bilge alarm and GE alarm light or beacon in generatior room are not working. Repair to the satisfaction of attending MI.

All generator alarm bells shall be identified by red lettering at least 1/2" "GENERAL ALARM - WHEN BELL RINGS GO TO YOUR STATION." The beacon and bell in the gen room were not labeled. Replace to the satisfaction of attending MI.

Pressure vessels that are required to be stamped must have the following: manufacture's name and serial #, Coast Guard #, Coast Guard symbol, MAWP, Class, min design temp & water capacity in liters. The #2 air receiver in the gen room has a wasted data plate. Renew or replace to the satisfaction of attending MI.

Class 1 doors are permitted within crew and working spaces wherever located if in the judgement of the OCMI the door is located where it will be closed at all times except when in use. Class 1 WTD located at frame 71 between the gen room and pump room cannot be closed. Repair to the satisfaction of the the attending MI.

Electrical equipment must be arranged as far as practicable to prevent mechanical damage to the equipment from dust, oil, steam or dripping liquids. The spring bearing cooler OVBD at frame 57 STBD side is leaking on top of a heater controller and another fuse/breaker panel or junction box. Repair to the satisfaction of the attending MI.

Complete but simple instructions for the operation of the system must be located in a conspicuous place at or near all pull boxes, stop valve controls. Paint locker fire extinguishing instructions are weathered/faded; clean or replace.

Within the accommodation and service area the following conditions shall apply:... Portable accommodation module (PAM) No. 1 has 02 improper electrical cable way penetrations through class A boundry.

Barges with sleeping accommodations for more than six persons...shall have permanently exhibited for the guidance of the officer in charge of the vessel... Fire control plan has not been updated to reflect current safety controls (ie. smoke detection, general alarm, and ESD, etc.).

No repairs or alterations affecting the safety of the vessel or its machinery shall be made unless applicable requirements in this chapter are met. Vessel reported they found a 2" diameter hole in the starboard store room bilge (FR 26) near the forward transverse bulkhead. Vessel is authorized 01 time transit to emergency dry dock for permanent repairs. 46 CFR 2.01-15(a)

The general alarm system shall consist of electric vibrating bells and in certain cases flashing red lights located throughout passenger and crew's quarters, machinery spaces, and so long as to warn all occupants in an emergency.Vessels general alarm did not sound in Chief Engineer and Captains stateroom when exterior doors were in the closed position. Old subchapter "J" 46 CFR 113.25-5(a)

Vessels on all routes 1600 GT's and over are required to have staorage batteries or diesel or gas turbine generator automatically or manual operated for a 12 hour operational period. Vessel crew was unable to start emergency generator for operational testing and to test alarms and shutdowns. OLd "J" 46 CFR 112.05-5(a)

Suitable guards shall be installed in way of exposed in dangerous places such as gears and machinery. Vessels dredge pump gears were exposed in machinery space posing a hazard to personnel. Old " I" 46 CFR 92-25-15(a)

When a manually started emergency lighting and power system is installed, the circuits listed in 46CFR 112.15-5,(embarkation station, passenger quarters, crew quarters, public spaces, machinery spaces shall be arranged so that they may be energized from the emergency source of supply. Vessel had emergency lighting throughout vessel that was inoperable and no lighting in machinery spaces. Old "I" 46 CFR 112.15-15(a)

No repairs or alterations affecting the safety of the vessel with regard to hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Vessel had an unapproved portable rental generator to replace missing required main generator. Vessel is required to submit plans for approval to use portable generator or replace original main generator. Old "I"

Installation of all systems of electrical engineering or interior communication nature, together with the details of design, construction and installation shall be in accordance with the requirements of subchapter "J". Vessel had junction box fwd upper machinery space that had wires pulled out of box and wire runs above emergency fire pump are being chaffed due to broken stuffing tube.

No repairs or alterations affecting the safety of the vessel with regard to hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Vessel had a soft patch on pipe coming from the main seachest to vessels main generator and all other onboard engines.

46 CFR 56.70 (c) Where fuel tanks are installed below weather deck, arrangements shall be provided for operating all shutoff valves at the tank outside the compartment where its located. Remote fuel shutoff valves from day tanks inoperable.

Each vessel must carry lifebuoys approved under approval series 160.150. Lifebuoy lines were deteriorated.

At least one-half the total number of lifebuoys on the vessel must each be fitted with a self-igniting light approved under approval series 161.010. Lifebuoy lights did not work and were replaced with wrong battery.

Exhaust pipe installations must conform with requirements of ABYC P-1 and part 1 section 23 of NFPA 302. Exhaust lagging on ship service diesel generator was missing.

There shall be at least two means of escape from all general areas accessible to the passengers if carried or where the crew may be quartered or employed. The emergency escape aft starboard side machinery space was seized and unable to be opened.

Fire hydrants shall be of sufficient number and located that any part of the vessel accessible to persons on board while the vessel is being navigated may be reached with at least 2 streams of water from separate outlets, one of which shall be from a single length of hose. A single length of 50' hose could not reach the portable accommodation modules.

The use of low melting point materials must not be used in vital systems unless approved by MSC. Vessel has plastic piping used in the fire main system.

No repairs or alterations affecting the safety of the vessel with regard to hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Vessels fire main supplies water to the accommodation modules. Produce approved fire main drawings showing this modification.

All general alarm bells shall be identified by red lettering at least 1/2 inch high: "GENERAL ALARM-WHEN BELL RINGS GO TO YOUR STATION" General Alarm in vessel's galley does not have required markings.

Alcohol testing devices need not be carried on board each vessel if obtaining the devices and conducting the required alcohol tests can be accomplished within 2 hours from the time of occurrence of the SMI. Vessel's on board alcohol testing strips expired in 2017. Vessel was unable to provide proof of available test kits within two hours.

a) For all vessels of 4,000 gross tons and over contracted for prior to January 1, 1962, existing structure arrangements and materials previously approved will be considered satisfactory so long as they are maintained in good condition to the satisfaction of the Officer in Charge, Marine Inspection. Minor repairs and alterations may be made to the same standard as the original construction. Major alterations and conversions shall be in compliance with the provisions of this subpart to the satisfaction of the Officer in Charge, Marine Inspection. Fire doors in the machinery space leading into the switch board do not latch lose, have missing mechanical latches and doors are rusted through. Fire boundary doors throughout vessel were also found not to latch properly.

Nameplates: Each tray shall be provided with a durable nameplate securely attached, bearing the manufacturer’s name or trade mark and type designation, the ampere-hour rating at a specific rate of discharge, and the specific gravity of the electrolyte. Date molded on the tray case will be acceptable in lieu of a nameplate. No information on battery trays in the switchboard room.

Ventilation: all rooms, lockers, and boxes for storage batteries should be arranged or ventilated to avoid accumulation of flammable gas. Poor ventilation in the stbd side switchboard room where the batteries and charger is located.

All marine engineering details such as piping, valves, fittings, boilers, pressure vessels, etc., and their appurtenances installed on the vessel, shall be designed, constructed, and installed in accordance with the provisions of subchapter F (Marine Engineering) of this chapter. Vessel's Firemain piping had hole in it.

All electrical engineering details and installations shall be designed and installed in accordance with subchapter J (Electrical Engineering) of this chapter. Extention Cord was wired in electrical distribution panel.

Each generator prime mover must have an overspeed device that is independent of the normal operating governor and adjusted so that the speed cannot exceed the maximum rated speed by more than 15 percent. MI was unable to test overspeed device due to failure in overspeed device.

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Vessel conducted repairs to CG approved Type II MSD.

A survey, either general, or partial according to the circumstances shall be made every time an accident occurs. Following report of damage to fore-peak area, submit temporary repair proposal to MI. iaw 46CFR91.30-1(a)

The survey shall be such to insure that the necessary repairs have been effectively made. Provide repair proposal and complete permanent repairs prior to credit drydock in 18Aug19. iaw 46CFR91.30-1(a)

Properly connect the hydrostatic release units to the inflatable liferafts.

Replace all expired floatlight batteries

Make operable port running light.

Properly mark EPIRB hydrostratic release unit with next service date.

Provide and install CO2 system instructions and diagram.

Provide test report for all bilge CO2 system bilge nozzles.

Provide CO2 warning placard throughout machinery spaces protected by CO2.

Provide updated MARPOL placards in engine room.

Properly terminate dead-end wiring in junction box or remove.

Remove permanent extension cords and power strips throughout vessel.

Make repairs to auxiliary generator cooling pump leaking shaft seal.

The auxiliary generator supply fuel hose may only be 30" in length. Provide and install hard pipe for section exceeding 30".

Submit plan and complete permanent repairs on fuel tank #8 prior to placing fuel in and placing tank back into service.

Complete the final repairs to the main pedastal bearing at the next vessel drydocking on 29Aug2016. All oil to be kept wiped up and maintained on regualr rounds.

1. To be completed prior to 30July2015 and prior to vessel operating, Provide accurate pressure gauge at the air receivers to replace the one that the engineer states is not accurate. This is needed for testing of the 4 air receiver relief valves that were not inspected from Activity 5037750. Tests to be witnessed by the USCG.

4. To be completed prior to 15Aug2015, Throughout the vessel, especially in the engineering spaces, remove all portable extension chords that have been installed and replace with proper cables and boxes they are too numerous to list all. Have examined by a USCG marine inspector.

5. To be completed by 15Aug2015, Prove all portable heaters meet 46CFR subchapter J part 111.83-7for the required manual reset overload

6. to be completed prior to 15Aug2015, Throughout the vessel and especially in the engineering spaces there are numerous areas where plastic wire ties are being used to hold up electrical cables in the overhead. Install proper metal bands to hold cables in overhead IAW IEEE 45 20.5 and 46CFR120.340 and have examined by a USCG marine inspector.

7. To be completed prior to 31 Aug 2015, Throughout the vessel check all electrical panel boxes to make sure all directories have the required correct information IAW 46CFR111.40-11. Have examined by USCG inspector when completed.

8. To be completed prior to 30July2015: In the engine room an unsatisfactory condition is present, a fire hazard excessive oil in bilges, excessive oil leaks being shielded by oil soaked sheets of cardboard, hoses, fittings, gaskets all need to be check for leaks and repaired.
