SAFETY AND HEALTH TRAINING: All training required in this section must be documented in owner or managing operator's records. During the inspection, master unable to find all records associated with Health and Safety training for annual refresher.

Resolution: P/E provided an updated and filled out refresher training record. The company created a training record which covers all of the items in 140.515, and keeps it with all other required items in the vessel's training binder.

Machinery and fuel tank spaces must be separated from accommodation spaces by bulkheads. An old penetration hole was found between the E/R and storage space above, just below pilothouse.
