Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of the persons on-board under normal and emergency conditions. The starboard diesel generator did not have any machinery guards installed to protect the crew from the forward belt drives. Repair in accordance with 46 CFR 58.01-20 to the satisfaction of an attending marine inspector. 46 CFR 143.205(a)

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. The starboard generator set was observed to be covered with oil outside and inside the protective covers presenting a fire hazard. Clean and repair to the satisfaction of an attending marine inspector. 46 CFR 143.400(a)

The Racor fuel filters installed on the port and starboard main engines are missing the bottom flame impingement bowls. Replace/install to the satisfaction of an attending marine inspector. 46 CFR 143.205(a)

Each towing vessel must be fitted with a general alarm that: has installed, in the engine room and other area where background noise makes a general alarm hard to hear, a supplemental flashing red light that is identified with a sign that reads:"Attention General Alarm-When Alarm Sounds or Flashes Go to Your Stations".The lower and upper engine room general alarms were not labeled. Label to the satisfaction of an attending marine inspector. 46 CFR 143.235(b)(3)

Each life buoy with a floating electric water light must have a lanyard of at least 910 millimeters (3 feet) in length, not more than 1,830 millimeters (6 feet), securing the water light around the body of the life buoy. Port side life buoy did not have any lanyard for the electric water light. Install to the satisfaction of an attending marine inspector. 46 CFR 141.360(c)(4)

The master must ensure that all lifesaving equipment is properly maintained and ready for use at all times. Distress signals expiration date is September 2022. Renew to the satisfaction of an attending marine inspector. 46 CFR 141.230

Propulsion machinery , including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must be maintained to ensure proper operation. Both MDE and Ship Generators displayed multiple leaks, exposed wiring/connections, inoperable gauge panels and F/O Leaks. Vessel is required to rectify systems to the satisfaction of the attending MI.

Piping and machinery components that exceed 220 °C (428 °F), including fittings, flanges, valves, exhaust manifolds, and turbochargers, must be insulated. Exhaust piping from both MDE and Ships generators displayed numerous areas of missing and damage insulation. Vessel is required to rectify systems to the satisfaction of the attending MI.

To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. The valve must be operable from a safe place outside the space where the valve is installed. When conducting test of fuel valves, valves were inoperable and must operate as designed. Vessel is required to rectify systems to the satisfaction of the attending MI.

All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. Bilges contained excessive Lube and Fuel Oil. Vessel is required to rectify condition to the satisfaction of the attending MI.

Each distribution panel or switchboard on a towing vessel must be In a location that is accessible, as dry as practicable, adequately ventilated, and protected from falling debris and dripping or splashing water. Distribution Panel was found saturated with fluids from adjacent systems. Vessel is required to rectify systems to the satisfaction of the attending MI.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Multiple breaker panels, breakers and switches are broken, open, or inoperable. Systems must function as designed. Vessel is required to rectify systems to the satisfaction of the attending MI.

Each towing vessel must have either a self-priming, power-driven, fixed fire pump, a fire main, and hoses and nozzles in accordance with paragraphs (a) through (d) of this section; or a portable pump, and hoses and nozzles Unable to test fire pump due to broken and unusable hoses, no spare available. Vessel is required to rectify condition to the satisfaction of the attending MI.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The following alarms must be provided:Low hydraulic steering fluid levels. Low Hydraulic steering alarm was found inoperable and in a tied open position using line. Vessel is required to rectify to the satisfaction of the attending MI.

At least one lifebuoy must have a lifeline, secured around the body of the lifebuoy. If more than one lifebuoy is carried, at least one must not have a lifeline attached. Upon inspection no line was found attached to life bouy. Vessel is required to rectify lifesaving appliances to the satisfaction of the attending MI.

Each lifebuoy with a floating electric water light must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the water light around the body of the lifebuoy. Floating Electric light found not attached to lifebuoy. (No existing line present at all). Vessel is required to rectify lifesaving appliances to the satisfaction of the attending MI.

Individuals, who are not crew members, on board a towing vessel must receive a safety orientation prior to getting underway or as soon as practicable thereafter, to include:The location, operation, and use of lifesaving equipment; Emergency procedures; Methods to notify crew members in the event of an emergency; and Prevention of falls overboard. Safety brief was not conducted/ and could not be provided upon request. Vessel is required to rectify safety program to the satisfaction of the attending MI.

The following list of items must be recorded in the TVR, official logbook, or in accordance with the TSMS applicable to the vessel:Personnel records, in accordance with §140.400; Safety orientation, in accordance with §140.410;Record of drills and instruction, in accordance with §140.420;Examinations and tests, in accordance with §140.615;Operative navigational safety equipment, in accordance with §140.620; Navigation assessment, in accordance with §140.635; Navigation safety training, in accordance with §140.645; Oil residue discharges and disposals, in accordance with §140.655; Record of inspection of towing gear, in accordance with §140.820; and Fire-detection and fixed fire-extinguishing, in accordance with §142.240. Vessel could not provide nor did they have active system in place to meet record keeping requirements. Vessel is required to rectify program to the satisfaction of the attending MI.
