Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must: (1) Be maintained to ensure proper operation. Vessel's port main engine suffered casualty resulting in engine shutdown. Diagnose and repair to the satisfaction of the Third Party Organization (TPO). Submit final TPO report to Marine Safety Unit Texas City upon satisfactory rectification.

Resolution: The repair is considered satisfactory per the attending Surveyor: Kim, Tokchung.

The TSMS must contain procedures for any person to report non-conformities. The procedure must describe how an initial report should be made and actions taken to follow up and ensure appropriate resolution. TSMS reporting procedures were not followed IAW the vessels SMS. Vessel was having steering issues while underway, and as a result, drydocked at Conrad Deepwater in Amelia, LA for repairs. As per repair report, three flanking rudders were found to be bent. These three rudders were cut, straghtened, and welded. Both main steering rudders were replaced. No report was made for damage creating an unsafe condition, damage resulting in reduced maneuverability, or when a marine casualty occurs that requires notification to the USCG.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Vessel lost propulsion when the Port engine leaked lube oil from the front of the engine. Technician reported that the engine's bearing had spun and found brass in the oil pan. Upon replacement of the Port engine, provide technician's report (on official letterhead) attesting to satisfactory tests and sea trials.
