A towing vessel of more than 26ft and more than 600 horsepower, engaged in commercial service must be equipped with a Class A Automatic Identification System Device (AIS). Vessel's AIS is inoperable. Vessel may not engage in commercial operations until repairs are made. Make repairs to satisfaction of TPO, Towing Vessel Inspection Bureau.

Non-tank Vessel Response Plan is required as follows: (a)Except as provided in paragraph (d) of this section, this subpart applies to each self-propelled vessel that - (1) Carries oil of any kind as fuel for main propulsion. Vessel does not have Non-tank Vessel Response Plan.

Each towing vessel must have either a self-priming, power-driven, fixed fire pump, a fire main, and hoses and nozzles in accordance with paragraphs (a) through (d) of this section; or a portable pump, and hoses and nozzles, in accordance with paragraphs (e) and (f) of this section. Fire pump does not provide proper preasure of 50 PSI.

The design of the system and its installation on the towing vessel is certified and inspected by a registered professional engineer with experience in fire-detection system design. The PE letter was not on board during exam.

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. Aft berthing areas UL217 battery opperated detectors not working.

Visual distress signals. Day and night visual distress signals were expired on vessel.

Installed electrical power source(s) must be capable of carrying the electrical load of the towing vessel under normal operating conditions. Generators would not stay on load for ships service.

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space. Aft steering room emrgency light did not work.

Pressure vessels over 5 cubic feet in volume and over 15 pounds per square inch maximum allowable working pressure (MAWP) must be equipped with an indicating pressure gauge (in a readily visible location) and with one or more spring-loaded relief valves. Both air receivers tanks need new safety relief valve.

A remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. The fuel shut off valve did not fully close for the stbd generator.

No active COFR

Prior to vessel being placed back into service. Provide TSMS required vessel survey and attestation from the Third Party Operator (TPO) that the vessel is in full compliance with the requirements of Sub-chapter M. 46 CFR 136.172

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford protection against hazards to life, property, and the environment. Vessel collided with foreign flagged deep draft vessel. Damage is on the port hull approx 45 to 50 ft long, set in approximately 2 feet. #4 port and #5 port fuel tanks have been breached with total fuel loss of 8954 gallons. Significant amounts of oil were found in bilge. Prior to vessel movement, submit a transit plan satisfactory to OCMI including receiving facility confirmation of availability. Prior to start of repairs, submit repair plan to satisfaction of OCMI. Prior to returning vessel to service, schedule repair check with OCMI.
