Electrical systems must minimize system failures, fire and shock hazards. Electrical equipment must be marked. Individual circuit breakers must be labeled. Electrical connections must be suitably installed to prevent them from coming loose through vibration or accidental contact. Electrical equipment and electric wires must be suitably protected from wet environments. Electrical components posing an electrical hazard must be in an enclosure. Electrical components passing through watertight bulkheads must be installed so that the bulkhead remains watertight. Connections of flex cable plugs must be designed to prevent unintended separation. There were numerous wires and cables in the bridge (below equipment panel) and in the engine room that were spliced, dead-end and exposed and not in a junction box, posing a fire and shock hazard. Wiring was loose and not suitably installed. Electrical connections were not labeled or marked. Vessel is required to put dead end/spliced cables in junction boxes or remove them. Vessel is required to ensure that cables and wiring are suitably affixed so as not to come loose, to include elimination of loose extension cords.
