If mechanical ventilation is provided in a protected space, the ventilation system must automatically shut down prior to the discharge of fixed fire suppression into that space. Four of the six mechanical ventilation systems failed to close upon actuation of the fixed fire suppression system. 46 CFR 34.01-5(a)

46 CFR 32.60-20: In tank vessels carrying grade A, B,C and D liquid cargo, cargo pumps shall be isolated from a source of vapor ignition by gastight bulkheads; the rubber gland in the cargo pump room bulkhead IWO the #1 pump shaft is deteriorated and compromised. The vessel is restricted from the carriage of flammable liquid cargo until corrected.

Scantlings, material and workmanship, subdivision of cargo spaces, fitting of cofferdams and testing of tanks shall be at least equivalent to the requirements of ABS. Following collision on 14NOV19, conduct survey to verify the material soundness of the vessel, prior to next dry dock. 705AC
