Should an incident occur, or a defect be discovered, which affects the safety of the unit or the efficiency or completeness of the structure, equipment, fittings, arrangements or materials, the person in charge or the owner of the unit should report the incident or defect at the earliest opportunity to the Administration... If the unit is in an area under the jurisdiction of another Government, the person in charge or the owner of the unit should also report the incident or defect immediately to the appropriate authorities of the port or coastal State and the nominated surveyor or recognized organization should ascertain that such a report has been made. The report referenced in deficiency no. 1 report was made to the coastal State five days after the damage was sustained and the lifeboat was out of service. At time of coastal State notification, Valaris acknowledged that the Administration had not yet been notified. Provide documentation on how notifications complying with this cite were made and if the Safety Management System sufficiently addresses how the "ship is maintained in conformity with the provisions of relevant rules and regulations". If determined to be non-compliant, provide intent on how future notifications are to be made "at the earliest opportunity to the Administration" and "immediately to the appropriate authorities of the port or coastal State". Cite: 09 MODU 1.6.6.3; 20 SOLAS IX/3.1; ISM 1.2.3.2, 10.1. Action: 16a,b,c.

Crew has not logged replacement of the parts/components which, when used in the engine will result in the continued compliance of the engine with the NOx emission limits in the EIAPP required technical files.

Following a reported marine casualty, marine inspectors identified structural damage to the starboard section of the vessel including but not limited to decks, vertical supports, overheads, and cross beams. PSCO requires vessel to be surveyed to assess damages and attest to repairs prior to departing the COTP Zone.
