Master reported that during fleet operation the winch was overloaded by torquing down too much. The port generator trip off line and shut down. The winch overload is a known existing problem. Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Make permanent repairs in that the all winches cannot be overloaded and trip either generator off line to the satisfaction of the attending Coast Guard marine inspector.

Each towing vessel must be able to test the set point of the low hydraulic steering fluid level alarm (alarm required by 46 CFR 143.230(a)(6)) and be recorded in accordance with part 140 of Subchapter M. No means to test set point and no record of set point test available at time of inspection. Provide TPO approved process for proper testing of low hydraulic steering fluid level alarm set point and recorded showing completion of test.

A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. Vessel was involved in an allision with the Peoria Lock causing damage to the starboard stern which has jeopardized the watertight integrity. The damage to the starboard stern has been temporarily patched and will be checked every hour and logged according to the vessel's transit plan. The vessel will transit, with tow, to approximately mm 168 of the Upper Mississippi River at the CGB Fleet St. Louis to offload barges and will travel light boat to Wood River, IL for permanent repairs. Vessel shall not engage in commercial service while transiting. Vessel will make permanent repairs to the satisfaction of the Coast Guard Marine Inspector

Make permanent repairs to the starboard main engine following issues of loss of propulsion reported to USCG. Vessel may move from MM 109.1 UMR to Wood River, IL for assessment/ repair. Repairs to be completed to manufacture specifications.
