Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must: (1) Be maintained to ensure proper operation; (2) Be suitable for route and service; and (3) Have suitable propulsion controls to provide the operator full control at each operating station. The crew reported that the vessel lost propulsion due to mechanical failure in the port main engine while underway, approximately 12 miles SW of Tinian. The starboard engine was reported to be operational. Make necessary repairs so that the port main engine is suitable for route and service. Contact USCG MSU Saipan for an examination of repairs. 46 CFR 143.205 (c)

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. While underway and shifting berths, vessel experienced a loss of pressure from the main air compressor and pneumatic delivery system for pneumatic steering, resulting in a loss of steering. After appropriate repairs are made, conduct sea trials to the satisfaction of attending Marine Inspector.

Towing vessels must be equipped with the following equipment, as applicable to the area of operation: (a) Fathometer. Fathometer onboard does not read correct depth either at the dock or underway. Additionally, fathometer is not operational as required under emergency battery power.

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space. There are no emergency lighting to illuminate the engine room's escape ladder to upper deck and laundry room to facilitate egress out in an event of an emergency.

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings in such spaces. The engine room port side exhaust dampener does not fully seal when in the closed position.

Towing vessels must be equipped with the following equipment, as applicable to the area of operation: Search light, controllable from the vessel's operating station and capable of illuminating objects at a distance of at least two times the length of the tow. Aft search light is not operational, vessel is restricted from towing operations at night.

Each owner or managing operator must develop procedures to routinely examine, maintain, and replace capstans, winches, and other machinery used to connect the towing vessel to the tow. The forward tow winch is not operational.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists, or an essential system develops problems that require attention. The high bilge level alarm for steering gear room is activated on the alarm panel on the bridge.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. The fire pump is missing the guard to protect personnel from rotating machinery.

The owner or managing operator must implement procedures to identify and mitigate health and safety hazards. The overhead light outboard of the starboard main engine is extinguished.

Towing vessels must be equipped with the following equipment, as applicable to the area of operation: Illuminated magnetic compass or an illuminated swing-meter (Western Rivers vessels only). The compass or swing-meter must be readable from each operating station. Compass light is extinguished and not visible during reduced visibility or during operations at night.

The master must ensure that all hatches, door, and other openings designed to be watertight or weathertight function properly. The pilot house starboard side quick acting watertight door’s upper dog is missing hardware and not properly tighten.

Electrical conductors passing through watertight bulkheads must be installed so that the bulkhead remains watertight. Two port side deck lightings on the 01 deck have wires passing through bulkheads that are not watertight.

Fixed fire-extinguishing system must be inspected and tested, as required by table 142.240 of this section. The paint locker fixed fire-extinguishing bottle’s bracket is severely wasted.

A towing vessel must carry adequate and up-to-date charts, maps, and nautical publications for the intended voyage, including: “U.S. Coast Pilot” or similar publication. The coast pilot onboard is not current.

Installed communication system between pilothouse, engine room and galley are inoperable. Communication with portable radio between engine room and pilothouse was not satisfactory, unclear and distorted.

Heat-detection system in the galley is inoperable.

Several fuel remote shut-off valves on exterior of main deck are unmarked.

Oil leaking from fitting flange inboard of STBD engine that is connected to exhaust pipe.

With AFT generator out of service, the panel for navigation light has only one source of electricity (generator). Panel is not connected to 12V nor the 24V systems.

Found flammable materials ( paints, primers) stored in engine room and through out the vessel.

VSL's engineer was not sure of how the essential loads are energized by two independent sources of electricity. VSL electrical system has been heavily modified with no electrical schematic drawing on board to verify .

A towing vessel must carry adequate and up-to-date charts, maps, and nautical publications for the intended voyage, including: Charts, including electronic charts acceptable to the Coast Guard, of appropriate scale to make safe navigation possible.

Any vessel over 300 gross tons using the navigable waters of the United States must have a valid COFR on board, the COFR on board was expired.

Each Towing vessel must be operated in compliance with the Maritime Transportation Security Act of 2002 and 33CFR 101-104. Vessel did not have an onboard master that was ready for the security assessment.

Vessel LLC has the incorrect call sign. LL symbol needs to be repainted on the side of the ship hull.

Each Vessel going on an international voyage must have no less than 12 rocket parachute flares approved under 46CFR160.136 and stow the flares on or near the vessel's navigation bridge.

Vessel is restricted from operating above 32 degrees north and below 32 degrees south latitude unless the appropriate amount of immersion suits are provided for the total persons on board plus an additional suit at each watch station, work station, and industrial work site.

Each lifebuoy on a towing vessel must be marked in block capital letters with the name of the vessel. The aft operating station buoy was not marked with the name of the vessel.

Each towing vessel operating in oceans and coastwise service must have a line throwing appliance approved under approval series 160.040. Vessel only had one out of 4 rockets and lines on board.

All portable fire extinguishers and semi-portable extinguish systems must be inspected or tested at least once every 12 months. Vessel portable and semi portable extinguishers did not have proper servicing documentation.

All flammable/combustible products kept on board must be kept to minimize fire hazard. Vessel stored flammable product outside of the paint locker.

Vessel did not have screen on the galley stove vent. Vessel shall minimize fire hazard on board. The Galley vent was catching grease well within the vent fan.

Each towing vessel must be equipped with an industrial first aid cabinet or kit, appropriate to the size of the crew and operating conditions. There were expired medical equipment on board.

The master must ensure that all hatches, doors, and other openings designed too be watertight or weather-tight function properly. The vessel had multiple WTD pins missing the cotter pin.

Vessels carrying oil as secondary cargo must carry appropriate equipment and supplies for the containment and removal of on-deck oil cargo spills of at least one-half barrel. Vessel oil spill response kit had missing equipment.

EACH TOWING VESSEL CERTIFICATED UNDER THE PROVISIONS OF THIS SUBCHAPTER MUST BE IN FULL COMPLIANCE WITH THE TERMS OF THE COI. THE ITV ENDEAVOR IS NOT PROPERLY MANNED AS PER THE VESSEL'S COI FOR THE REQUIRED DECK OFFICERS (MASTER/MATES) HAVING THE PROPER LICENSE/ENDORSEMENT AS PER 46CFR15.910.

The owner, master, or operator of each vessel towing astern shall ensure that the strength of each towline is adequate for its intended service. Tow wire parted while the vessel was conducting towing operation. Vessel is to affect permanent repairs or provide a tow wire for the intended towing operations to the satisfaction of the attending CG MI prior to conducting astern towing operations.

There must be an installed or portable bilge pump for emergency dewatering. Any portable pump must have sufficient hose length and pumping capability. All installed bilge piping must have a check/foot valve in each bilge suction that prevents unintended backflooding through bilge piping. There is a leak in the bilge piping manifold, make repairs to the piping system prior to movement.

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. Replace the expired contents of the first aid kit.

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. Replaced the expired eye bottles in the ships main interior passageway.

Towing machinery used to connect the towing vessel to the tow must be suitable for its intended service. It must be capable of withstanding exposure to the marine environment, likely mechanical damage, static and dynamic loads expected during intended service, the towing vessel's horsepower, and arrangement of the tow. The vessels starboard aft tow winch was not operational at the time of the inspection.

To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. Although the starboard aft fuel oil storage tank does not supply fuel directly to a propulsion engine or generator prime mover, the remote mechanism has been removed and needs to be replaced as designed.

The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. Make repairs/adjustments to the port and starboard watertight doors in the pilot house. Both doors require the use of extension bar to open and close normally and ensure that they are weather-tight.

Although the 3 center fuel oil storage tank does not supply fuel directly to a propulsion engine or generator prime mover, the remote mechanism has been removed and needs to be replaced as designed.

At least two lifebuoys on a towing vessel greater than 26 feet must be fitted with a floating electric water light approved under approval series 161.010 or 161.110, unless the towing vessel is limited to daytime operation, in which case no floating electric water light is required. The batteries in both floating electric water lights were expired.

Service all portable extinguishers onboard IAW 46 CFR 142.240(a).

Install a man over board recovery device IAW 46 CFR 141.230.

Make repairs to the excessive water ingress through the rudder post packing IAW 46 CFR 143.205(a).

Replace gasket for the port side pilot house water tight door IAW 46 CFR 144.320(b).

Diagnose and make repairs the the starter / gearing issues on both main propulsion engines IAW 46 CFR 143.205(a).

Make permanent repairs to the temporary soft patch on the bilge / ballast piping system IAW 46 CFR 143.245(a).

Each water tight door must be marked on both sides in 1 inch letters "WATERTIGHT DOOR-KEEP CLOSED".

Provide certification for pressure vessel gauges.

Complete testing of pressure vessel relief valves.

Secure air leak at shaft brake valve stem.

Hard wire emergency lighting in engine room.

Repack port side shaft packing.

Provide check valves at all bilge foots. Starboard engineroom bilge piping missing check valve.

Provide guard for spinning machinery on dirty oil pump.

Make engine room emergency escape ladder stable and make lower rungs avalible.

Test main engine and aux generator low lube oil and high temperature alarms.
