No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment shall be made without the knowledge of the OCMI. Found a temporary repair with splash zone on the turn from the head log to the main deck of the forward center rake. Submit repair plan for review/approval prior to making permanent repairs.

Side lights each showing an unbroken light over an arc of the horizon of 112.5 degrees and so fixed as to show the light from right ahead to 22.5 degrees abaft the beam on its respective side. Port and starboard side light's post mount is bent and not displaying the proper angle of view.

The ring, lines, and letters shall be painted in white or yellow on a dark ground or in black on a light ground. The load line plimsoll mark and deck line is unreadable.

Replace missing hardware for manhole covers and sheered off bolt on multiple multiple manhole covers and repair aft most pad eye on starboard main deck in way of stbd aft rake.

Suspected bottom plating wastage in forward port rake. Remove water and gage bottom plating in way of ladder. Submit gage report to USCG for review and effect repairs if required to the satisfaction of the atending MI. Updated 13MAY2022: PDATE/13MAY2022: Approval granted by Sector Guam's CID to clad weld pits gaged over 25% on bottom hull plating in forward center rake by no later than 01JUN2022, to the satisfaction of the attending USCG Marine Inspector (MI). In addition, space must be certified gas free by a Marine Chemist for USCG MI entry for further inspection. Note: bottom hull plating in forward center rake is subject to further repairs.

Port side deck line with wastage. Repair port side deck line to as original.

Replace wasted upper flat bar attached to ladder with new in port aft center rake at next dry dock or ISE, whichever comes sooner.

Based on log book, found that vessel has been operating with an invalid IOPP Certificate since 28 DEC 2018.

Take random gauging of deck on barge 5 feet aft of midship portside from edge inboard 15-18 feet. Approx 4-8 shots to compare against original scantlings.

S-3 void finish weld in overhead, S-1 void finish circle plate repair in the overhead, Port fwd peak frame seperated from hull needs repair, repair fractured stbd pad eye for towing bridle.

Guaging of lower aft rake collision bulkhead to determine % of wastage is to be conducted NLT 04APR98. Guaging to be conducted at most wasted areas in lower 18 inches of bulkhead at intervals not to exceed 18 inches. If more than 25% plate thickness missing, crop & renew to sound plate.

Collision bulkhead in way of the port ladder in the forward center bow rakeis deteriorated & requires renewal. Est. size of required insert is 6.5 feet x 2.0 feet (H x W). Permanent repair to be made not later than 04APR98after a repair proposal is submitted to the USCG Office conducting oversite

BATTERY BOXES must be lined or replaced with boxes of approved material. This includes one on the bow and two in the generator room.

MAKE CONTAINERIZED GENERATOR SET AVAILABLE FOR INSPECTION. INSPECTION SHALL INCLUDE BUT NOT LIMITED TO; GENERATOR TRIPS, ELECTRICAL SYSTEM, AND FIREFIGHTING SYSTEM.

PROVIDE MSO HONOLULU A CERTIFIED COPY OF THE VESSEL'S STABILITY LETTER.

PROVIDE MSO HONOLULU WITH A COPY OF THE DOCUMENTATION CERTIFICATE.
