Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of the persons onboard under normal and emergency conditions. While shifting barges, VSL hit a shoal area resulting in the rudder being pushed into the propeller(damage to the rudder and propeller) VSL is to effect repairs, have TPO/ABS survey and test the VSL (Sea Trials)and submit all reports to the attending Marine Inspector. Vsl shall not move until deficiency is rectified. 46CFR143.205(a) 60C

Resolution: ABS provided report stating that Port Propeller, Port Tiller Arm, Jockey Arm, Hydraulic Steering Rams, Port Rudder and Port Rudder Shaft recieved damaged and have been repaired to ABS satisfaction.

The functional requirements of a TSMS include: Procedures for reporting accidents and non-conformities. On 19AUG20 at 1330 VSL hit a shoal while shifting barges at mm 103 Westwego, resulting in rudder(steering) and propeller( propulsion) damage. VSL did not report the incident until 1400 20AUG20. VSL must submit TSMS reporting procedures showing appropriate compliance and which incidents need to be reported. VSL shall not move until documentation of TSMS reporting procedures are shown to the attending Marine Inspector 46CFR138.215(c) 60C
