Issued 21 September 2021 Resolved
Machinery and electrical systems must be designed and maintained to provide safe operation of the towing vessel. Starboard main engine failed due to fire in upper engine room. Repair or replace Starboard engine to the satisfaction of the TPO or cognizant OCMI.
Action required: 60 - Rectify deficiencies prior to movement

Resolved 15 October 2021
Resolution: Both main engines were replaced with Caterpillar C18 diesel engines. Both main engines were tested and inspected to the satisfaction of the TPO. See attached TPO report for further details.
