The cognizant OCMI may require additional examinations of the vessel whenever he or she discovers or suspects damage or deterioration to hull plating or structural members that may affect the seaworthiness or fitness for the route or service of a vessel. Vessel was involved in a marine casualty and received damage to the headlog and bow void as follows:
Headlog set in approximately 4" over an eight foot area with deck, bottom plate, bulwarks and internal framing distorted. Starboard bow hatch can not close due to deck buckling. Vessel authorized to transit from Belle Chase launch to Conley Marine Services in [name withheld] for permanent repairs. Once at shipyard, prior to movement, make permanent repairs to the following:
Fender 12" x 24" x 16', Headlog 3/4" x 30" x 8'
Deck Plate 3/8" x 2'9" x 7', Bulwark 5/16" x 1'6" x 6'
Bulwark Cap 3/8" x 3" x 6', Headlog transverse stiffner 3/4" x 10" x 8', Headlong bottom stiffner 3/4" x 18" x 5', Transverse bottom angles 3" x 5" x 5/16" over 7'
Make permanent repairs to the satisfaction of TPO. Provide post repair report to USCG Sector New Orleans.

Machinery and electrical systems must be designed and maintained to provide safe operation of the towing vessel and safety of persons onboard during normal and emergency conditions. Vessel experienced loss of STBD engine resulting in Marine Casualty. In addition vessel has 3" crack in port bow void and the STBD propeller shaft backed out of the coupling. Vessel is currently moored alongside Stone Oil facility at Mississippi River, 96.5 mile marker.
Action 705: Vessel is authorized direct transit with assist tug, as per submitted transit plan, from Stone Oil facility to Bollinger Quick Repair in [name withheld]. Upon arrival, conduct full survey of vessel to identify all permanent repairs needed and communicate repairs to OCMI. Prior to departure from Bollinger Quick Repair, vessel shall provide a technician report attesting to cause and satisfactory repair to original design. 46CFR143.205(c)(1)
