46 CFR 143.205(a) Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Integral keel cooler leaking. Make permanent repairs to keel cooler, in kind.

Piping and machinery components that exceed 428 degrees Fahrenheit, including fittings, flanges, valves, exhaust manifolds, and turbochargers, must be insulated. Main engine turbocharger and flex coupling missing insulation/lagging. Renew lagging around exposed sections of both main engines.

An exposed hazard such as gears and rotating machinery, must be protected by a cover, guard or rail. The starboard generator accessory drive pulley is missing a protective guard. Renew guard to fully cover rotating machinery hazards.

No person may operate any vessel equipped with installed toilet facilities unless it is equipped with: (1) An operable Type II or III device that has a label on it. MSD onboard missing label. Affix new label/placard.

Decks and bulkheads designed to be watertight or weathertight must be maintained in that condition. Forward machinery space has a bulkhead penetration via pipe, below the bilge manifold piping. Close the penetration to ensure watertight integrity.

When conducting an examination of a towing vessel, the surveyor must determine whether any defect, deterioration, damage, or modifications of the hull and related structure and components may adversely affet the vessel's seaworthiness, fitness or suitability for route and service. Vessel shall not conduct any commercial operations until the following items are repaired: 1. Crop out and renew side shell plating and any damaged internal framing on port and starboard stern quarters. 2. Crop out and renew hull plating and any damaged internal framing on headlog. 3. Crop out and renew damaged plating on fuel tank bulkhead. 4. Repair or replace port and starboard propellers. 5 Renew port and starboard tail shaft bearings. 6. Pull port and starboard tail shafts and make available for Coast Guard inspection. 7. Renew rudder post bearings on port and starboard steering rudders. 8. Repair cracks in port and starboard rudders. 9. Repair port and starboard struts. 10. Repair damaged area of integral keel cooler. Coast Guard is required to witness all cut outs, fit ups, back gouges, and final welds. Non Destructive Testing is required for all inserts on hull and fuel tank plating. Vessel is authorized to transit to National Maintenance at MM 949 LMR to conduct permanent repairs to the satisfaction of attending USCG Marine inspector.

When conducting an examination of a towing vessel, the surveyor must determine whether any defect, deterioration, damage, or modifications of the hull and related structure and components may adversely affet the vessel's seaworthiness, fitness or suitability for route and service. There are sharp insets to the hull plating in the port and starboard steering voids. There are improperly repaired cracks/tears in port side shell plating, in way of the sharp inset, one of which is weeping water. Inspectors also identified that the fwd bulkhead of the starboard fuel tank is deflected where the bulkhead meets the bottom shell. Vessel is required to be dry docked within 30 days to make repairs. Repairs are to be approved and witnessed by the attending Marine Inspector.

Each towing vessel, if applicable, must be fitted with a low hydraulic steering fluid level alarm. Vessel is not equipped with a low hydraulic steering fluid level alarm.

Alarm set points are required to be tested twice every five years, with no more than three years elapsing since last test. Vessel was unable to test the set point on the high cooling water temperature alarms serving the propulsion engines or generator prime movers. Testing of the alarm set points are required to be witnessed by attending Marine Inspector.

"A towing vessel subject to this sub-chapter and choosing the Coast Guard inspection option, must undergo an annual inspection within 3 months before or after the COI anniversary date." Vessel has not had an annual inspection within the alloted window. Make vessel ready for inspection immediately.

Vessel must carry a 160B semi-portable fire extinguisher to protect the engine room. The 160B was on the second deck and the hose would not reach all spaces of the engine room. Ensure hose can reach all areas of the E/R.

Vessel must be maintained and operated to minimize fire hazards. Vessel had multiple fuel hoses that showed significant chaffing of the following non-metallic fuel hoses: fuel line from RACOR to port engine, one line on the starboard main engine, and one on the starboard generator. Replace hoses and test.

Machinery must be maintained to provide for safe operations of the towing vessel and safety of persons onboard under normal and emergency conditions. An exhaust leak was observed on the starboard side of the port engine near the turbocharger. Fix or replace the turbo.

Emergency lighting must be provided for all internal crew working and living areas to provide sufficient illumination under emergency conditions to facilitate egress from each space. The vessel did not have adequate emergency lighting on the top of the engine room and the galley's emergency lighting did not function. Provide proper E-lighting in each location.

Each space must have two means of escape. The starboard side wooden galley door did not have a handle or way to allow easy egress. Install a handle on the door.

Vessel must have means to monitor and control the position of the rudder at each operating station. Vessel did not have a rudder angle indicator and was not able to see the position of the rudder from the operation station. Install a rudder angle indicator on the pilot house nav panel.

The officer in charge of a navigation watch must conduct a navigation assessment for the intended route and operations prior to getting underway. Navigation assessments are not being conducted as required. Conduct navigation assessments prior to getting underway from B&J Dock.

High bilge level alarms must be visible and audible at each operating station. The bilge alarm is not visible and audible in the wheelhouse. Repair the audible and visible bilge alarm and prove proper operation to the satisfaction of the attending marine inspector.

A towing vessel must record the items listed in 46 CFR 140.915. The vessel does not contain a record of events as required. Vessel must maintain proper record keeping in accordance with 46 CFR 140.910 and 140.915.
