03 - Water/Weathertight Conditions › N/A - No Subsystem › Machinery space openings
Issued 26 January 2019
Resolved
The master must ensure that all hatches, doors and other openings designed to be watertight or weathertight function properly. Id'd fuel shut off recess and adjacent piping (port&stbd sides) is not watertight to the super structre. Vessel to re-configure recess without diminishing fuel-shut-off functionality to ensure the recess is watertight.

Issued 25 January 2019
Resolved
All bilges and void spaces are kept free from
accumulation of combustible and flammable materials and
liquids insofar as practicable. Identified 2-3 inches of
fuel in the bilge spaces beneath the port & starboard
engines, port and starboard generators, and fuel

Issued 25 January 2019
Resolved
The master must ensure that all hatches, doors, and
other openings designed to be watertight or weathertight
function properly. Identified three watertight hatches
and hatch frames on the main deck (2 aft port and
starboard, 1 in the rudder room), were excessively worn
and rusted preventing the proper watertight closure,
creating a serious substandard condition not being proactively managed by the Company, vessel owner, and/or
operator. 46 CFR 140.610(b)
Action required: 30 - Ship detained
Resolved 26 January 2019
Resolution: Witnessed satisfactory operation of all watertight doors. Verified replacement and weld tested main deck hatches.
13 - Propulsion and Auxiliary Machinery › N/A - No Subsystem › Operation of machinery
Issued 25 January 2019
Resolved
Machinery and electrical systems must be designed and
maintained to provide for safe operation of the towing
vessel and safety of persons onboard under normal and
emergency conditions. Identified excessive shaft leakage
on starboard propulsion shaft with rigged means of
dewatering with pumps to maintain leakage, creating a
serious substandard condition not being pro-actively
managed by the Company, vessel owner, and/or operator. 46 CFR 143.205(b)
Action required: 30 - Ship detained
Resolved 26 January 2019
Resolution: Shaft seal was tightened, reducing leakage drastically.
03 - Water/Weathertight Conditions › N/A - No Subsystem › Doors
Issued 25 January 2019
Resolved
The master must ensure that all hatches, doors, and
other openings designed to be watertight or weathertight
function properly. Identified the watertight door on the
bow, and starboard aft watertight door in the engine
room were missing large portions of the gaskets
preventing the watertight integrity of those spaces. To
be completed upon USCG review and approval of transit
and repair plan. 46 CFR 140.610(b)
Action required: 17 - Rectify deficiencies prior to departure
