Vessel starboard engine keel cooler has an unauthorized epoxy patch repair and additional pin hole leaks. Machinery and electrical systems must be designed and maintained to provide safe operations of the towing vessel and safety of persons onboard under normal and emergency conditions. Conduct satisfactory repairs to keel coolers prior to carrying passengers and prior to departure to the satisfaction of the attending Coast Guard marine inspector.

Life jackets did not have proper marking of location, missing whistle and light. Life jackets must be readily accessible and the storage container must be marked in block capital letters with quantity. Each life jacket must have a whistle and a light approved under 161.012 or 161.142. Mark/label Life jacket storage with LIFEJACKET and list quantity. Provide each life jacket with a whistle and approved light prior to movement of the vessel to the satisfaction of the attending Coast Guard marine inspector.

Vessel does not have an installed fire detection system. Each towing vessel must have a fire detection system installed to detect engine room fires. Each detector control panel and fire alarm are approved by the Commandant under approval series 161.002 or listed by a NRTL as set forth in 29CFR1910.7. Install approved fire detection system as per 46CFR142.330 to the satisfaction of the attending Coast Guard inspector.

Rudder angle indicator electrical connection on stern of vessel has electrical tape securing the plug connection. Electrical connections must be suitably installed to prevent them from coming loose through vibration or accidental contact. Electrical cables must be suitably protected from wet and corrosive environments. Secure electrical connection with heat shrink for protection from weather to the satisfaction of the attending Coast Guard marine inspector.

The bolts and nuts to the ladder leading from the galley to the pilot house are rusted or have fallen off. The ladder is currently being held in place by wire on one side. The owner must implement procedures to identify and mitigate health and safety hazards including slips, trips and falls. Install ladder as designed with appropriate nuts and bolts to the satisfaction of the attending Coast Guard inspector.

Provide an additional fire hose with quick connect fittings that meets the requirements set forth in 46 CFR 142.325 that can readily be deployed and connected with one of the two (02) hoses already in place on the QUEEN, in the event of a fire on board the ITV JANET E. The hose must be lined commercial fire hose 50 feet in length, at least 1.5 inches in diameter, and fitted with a nozzle made of corrosion resistant material capable of providing a solid stream and a spray pattern.
