Vessel did not receive a dry dock exam by the October 31 2023 deadline on its COI. The owner or managing operator shall make a vessel available for drydock examinations, internal structural examinations, and underwater surveys required by this section. Fire Main had two leaks, one fitting on main deck and one at the fire plug on the second deck. All piping, valves and fittings shall be accordance with good marine practices and suitable for its intended purpose. Make repairs to the satisfaction of the attending Coast Guard marine inspector

Ring Life Buoys name of vessel were missing or faded. Each ring life buoy must be marked in clearly legible block capital letters with the vessel name. Make correction to the satisfaction of the attending Coast Guard marine inspector.

Owner did not have crew available to perform drills during the annual inspection. The scope of the annual inspection is the same as the inspection for certification but in less detail The owner or operator must conduct all tests as required by the OCMI, and make the vessel available for all specific inspections and drills required by subpart H of this part. In addition, the OCMI may require the vessel to get underway. Perform fire, man overboard and abandon ship drills prior to carrying passengers to the satisfaction of the attending Coast Guard marine inspector.

Master has not performed underway fire drills for Coast Guard Marine inspector. The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. Master shall perform underway fire drills to the satisfaction of the attending Coast Guard Marine inspector prior to 03 Aug 2022.

Master has not demonstrated underway navigation which include vessel handling characteristics in presence of Coast Guard marine inspector. The master shall operate the vessel keeping the safety of the passengers and crew foremost in mind by directing the vessel in order to preventing a marine casualty. Master shall demonstrate vessel navigation to the satisfaction of the attending Coast Guard marine inspector prior to 03 Aug 2022.

Vessel fire hose nozzle are plastic are not the required type. Each combination solid stream and water spray firehose nozzle required to be approved under the provisions of this subpart must be of brass or bronze, except for hardware and other incidental parts, which may be of rubber, plastic, or stainless steel, and designed, constructed, tested, and marked in accordance with the requirements of ASTM F 1546 or NFPA 1964. Provide fire hose nozzle in accordance with 46 CFR 118.320 to the satisfaction of the attending Coast Guard Marine inspector.

Owner failed to provide training records for crew. The owner, charterer, master, or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 122.510, when applicable, the duties listed in the station bill required by § 122.514. Provide records of crew training to the satisfaction of the attending Coast Guard marine inspector.

Vessel bilge system was not operational during inspection. A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Make repairs to bilge systems and make available for inspection to the satisfaction

Owner failed to provide records compliance with drug and alcohol program, pre-employment testing and random testing. Provide drug and alcohol testing requirement to the satisfaction of the attending Coast Guard marine inspector.

Forward number 1 void hatch was leaking due to lack lack of maintenance on hatch cover and gasket. Each hatch exposed to the weather must be watertight. Conduct maintenance and repairs to number 1 void hatch to the satisfaction of the attending Coast Guard marine inspector.

Master has not demonstrated power bilge pumps driven by two generators. If both of the required fixed power bilge pumps are electrically driven two generators shall be provided. Maser shall demonstrate bilge pump

Vessel does not have written senior deckhand policy/training demonstrating increased knowledge and skill of senior deckhand as required by the the Certificate of Inspection. Training and qualification needs to be commensurate with the duties of the senior deckhand duties. Provide senior deckhand policy/training in accordance with NVIC-91 to the satisfaction of the attending Coast Guard marine inspector.

Number 2 void bilge valve on the bilge manifold has a pin whole leak preventing de-watering of the space. Every vessel must be provided with a satisfactory arrangement for draining any water tight compartment. Replace bilge manifold vale to the satisfaction of the attending Coast Guard Marine Inspector.

Each ring life buoy with a floating waterlight must have a lanyard of at least 3 feet in length but not more than 6 feet, securing the waterlight around the body of the ring life buoy.

Each life jacket kept in a storage container must be readily available. Replace rusted latch on the aft storage container, 2nd deck.

Remove and/or replace expired items in the first aid kit and ensure that the kit meets the minimum requirements in 46 CFR 160.041 (attached) or an equivalent.

B-II fire extinguisher did not have charge.

Each life jacket kept in a stowage container must be readily available. Latches on top deck have been painted over preventing them from opening.

Any hatch exposed to the weather must be watertight IAW 46 CFR 116.1160
