Pressure vessels over 5 cubic feet and 15 PSI must be equipped with atleast one spring-loaded relief valve with a relieving capacity sufficient to prevent pressure from exceeding MAWP by no more than 10 percent. Relief valves must be tested twice in 5 years, not to exceed 3 years between tests. The main air receiver was not tested or recorded as being tested/replaced. The secondary air compressor had a MAWP of 150PSI with an 170PSI relief installed, excedding 10 percent. Replace/test/record Pressure relief valves.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard. Vessel’s steering system was found to have excessive hydraulic fluid on the equipment and surrounding area. In addition, a new limit block was incorrectly installed (welded in wrong position) on the steering system – not allowing for proper operation of the system. Clean hydraulic fluid and repair any leaks that are detected. Relocate incorrectly positioned limit block to allow proper operation of steering system.
