No person may operate a non-oceangoing ship of 100 gross tons and above that is fitted with main or auxiliary machinery spaces in the navigable waters of the United States unless:(1) The ship has at least one pump installed to discharge oily mixtures through a fixed piping system to a reception facility;(2) The piping system required by this section has at least one outlet that is accessible from the weather deck;(3) Each outlet required by this section has a shore connection that is compatible with reception facilities in the ship's area of operation; and (4) The ship has a stop valve for each outlet required by this section. Vessel does not have fixed oily mixture discharge system installed on board. 33 CFR 155.410

To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. The valve must be installed in the fuel piping directly outside of the fuel oil supply tank. Valve for main diesel tanks is not in the correct location directly outside tank. 46 CFR 143.260(b)

Towing vessels must be equipped with the following equipment, as applicable to the area of operation: Illuminated magnetic compass or an illuminated swing-meter (Western Rivers vessels only). The compass or swing-meter must be readable from each operating station. Vessel is not fitted with magnetic compass or swing meter. 46 CFR 140.725(d)

The instructions supplied by the manufacturer must contain directions for each of the following: Installation of a vent or flue pipe. Ventilation for MSD onboard vessel is not in accordance with manufacturers instructions. 33 CFR 159.57(a)(4)

The portable fire pump must be self-priming and power-driven, with:(1) A minimum capacity of at least 300 LPM (80 gpm) at a discharge gauge pressure of not less than 414 kPa (60 psi), measured at the pump discharge; Portable fire pump on board does not have installed gauge at discharge.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. The Stbd. hydraulic steering hoses/hard lines are resting on the STBD steering ram. Mount/secure hydraulic lines to clear the steering ram to prevent chaffing.

Individual circuit breakers on switchboards and distribution panels must be labeled with a description of the loads they serve. The aft engine room center breaker panel cover was replaced and is missing the proper labeling. Label breakers with a description of the loads they serve.

Each remote valve control must be marked in clearly legible letters, at least one inch high, indicating the purpose of the valve and the way to operate it. The vessel's remote fuel pulls were not marked. Mark remote fuel pulls IAW the regulation and provide evidence to the attending MI.

Vessel is required to have applicable transfer procedures due to oil carrying capacity. Vessel did not have applicable procedures. Provide applicable transfer procedures and subsequent line diagram for the vessel.
