Propulsion Machinery must be maintained to ensure proper operation. Hydraulic hoses on MDE reduction gear are wasted beyond serviceable life and pose a hazard to personnel, vessel and port.
Each towing vessel must be fitted with a general alarm. Alarm has been inoperable for at least two weeks with no apperant attempt at repair from company.

Towing vessels must have a reliable means to provide notification when an emergency condition exists a high bilge level alarm must be provided. the installed Engine room bilge alarm was fitted with a disconnect switch located in the engine room. During vessel operation the alarm was turned off, demonstrating a disregard to the required safety feature. Disconnect switch must be removed.

All towing vessels must operate in a manner that minimizes the risk of downflooding and progressive flooding. Aft void spaces contain excess water. Master states water comes from rudder post and is pumped weekly. Could not produce any records/logs attesting to pumping void and during normal operation visual inspection of rudder post witnessed no water coming out into the space.
